Hello, thanks so much for your interest in joining the Fiverr Empower program for freelancers. Please fill out this registration form to tell us a bit about you.

Once the form is submitted, a member of the Fiverr Empower team will be in touch with you to let you know if you have been accepted into this program. They may also reach out to you with further questions.

What does the registration form include and ask you to do?

• First and Last Name
• Email address
• How long you have been a freelancer
• What your freelance skill sets include
• How many hours a week you are looking to take on freelance work
• Your accessibility needs and assistive technology requirements
• The registration form will require that you upload a resume and/or samples of your work in addition to providing any online links to view your work
• Contact information and time zone

The registration form will also request that you please answer the following two questions either by writing a response, audio recording a response or creating a video of yourself talking through your response:

1. Why would you like to join the Fiverr Empower Program and become a freelancer on the Fiverr Platform?

2. What previous and/or relevant freelance experience do you have that will help you be a successful freelancer on the Fiverr platform?

How will your information be used and stored?
Fiverr and Open Inclusion comply with the European Union's General Data Protection Regulation (GDPR)—the highest privacy standard. Open Inclusion will collect data on this registration form and share it with Fiverr for the purposes of connecting you to the Fiverr Empower program only. Your data will never be shared with a third party or used for unsolicited marketing outside the Fiverr Empower Program.

Any data Open Inclusion collects through this registration form will be stored on a secure and password-protected file system and deleted within two years unless you specify otherwise. Any data you share with Open Inclusion through this registration form may be shared with Fiverr for the purpose of contacting you for the Empower Program. Your information will not be shared for any other purpose including marketing. If you have any questions about how or where your personal data is being stored, used, or wish to delete any data Open Inclusion holds, please contact us at contact@openinclusion.com.

Please answer the following two questions by creating a short video, creating a short audio recording, or by writing a short response:

1. Please tell us why you would like to be a freelancer on the Fiverr platform and why you would like to be a part of the Fiverr Empower program.

2. Please tell us about any relevant freelance experience you have that would make you an effective freelancer on the Fiverr platform.

You can choose one of the following formats that best works for you to respond to the two questions above:

Film a short video of yourself (less than 5 minutes)

or

Create a voice recording of yourself (less than 5 minutes)

or

Write a short response (max 600 words)

Submitting a video or audio response

Please make sure the whole video is no longer than 5 mins and uploaded as an MP4 or MOV file. Your video can be up to a maximum of 5 minutes long. However, you do not need to use all of the 5 minutes.

• You can record your video in multiple files. Please do not upload more than four files. The four files must not exceed a total of 5 minutes. If they do, it will void your application.

• Please record in SD or HD (not 4K)

•  Ensure your video files are no more than 120MB.

•  Please shoot the video in landscape mode (if possible)

• You can record the video with your phone camera or you can record yourself with your phone’s built-in voice recorder app (like Voice Memos), then email it to your computer to upload on the form.

Tips for submitting a written response

• Please make sure your written response is no longer than 600 words and is uploaded as a PDF or Word Doc file.
