I recognize the exercise might be difficult and strenuous and that there could be danger inherent in exercise for some individuals acknowledge that the possibility of certain unusual physical changes during exercise does exist. These changes include abnormal blood pressure, fainting disorders in heartbeat, heart attack, and in rare instances death l understand that as result of my participation in an exercise, could suffer an injury or physical disorder that could result in becoming partially or totally disabled and incapable or performing any gainful employment or having normal social life recognize that prior to involvement in any exercise program, participants should obtain an examination by a physician. if have chosen not to obtain physician's permission prior to beginning the exercise program with any and all other fitclub coaches, hereby agree that am doing so at my own risk. - acknowledge and agree that no warranties or representations have been made to me regarding the results will achieve from this program I understand that results are individual and may vary [CHECK BELOW TO ACKNOWLEDGE]

Waiver and Covenant Not to Sue . We require you to wear appropriate clothing and footwear while in its facilities such as gym shorts, T-shirts (that cover your chest), undergarments or tights with all apparel jogging aerobic and sweat outfits Street clothes/shoes and jeans are not appropriate This is a family environment Please be sure to keep tops on and wear modest clothing attire Being barefoot in the general workout area is prohibited Live Active Nutrition recommends closed toed shoes be worn on the general workout all times, and wearing opened toed shoes or socks alone is at your own risk.
