    Please fill this form to help me better understand your hair goals and expectations. We will go over these questions at your in person consultation when I have the opportunity to see your hair in person. By the time you leave, I want you to feel confident in the plan we create for your hair and know exactly what to expect at your appointment. We will schedule your color/extension appointment at the time of your in person consultation to ensure the proper timing is booked. Don't stress about your answers, this is not a test and they don't have to be perfect. Once you have submitted your form, you will be redirected to a link to book your consultation online at your convenience.

    • The client is aware that the following are all factors in the outcome of the service:

      • Hair type
      • Genetics
      • Allergies
      • Hormones
      • Medications
      • Previous chemical services (professional or at home)
      • At home care
      • Minerals from hard water and copper pipes

      It is the clients responsibility to disclose all relevant information and hair history to the stylist.

      Even when disclosed, the stylists cannot guarantee the outcome of any service. Reactions are unpredictable even after multiple sessions of using the same products and procedures.

      The stylist will use their knowledge and expertise to proceed with caution when performing services.

    • At Home Hair Care & Maintenance 
    • The client is aware that the longevity of the service and/or extensions is dependent on maintaining the recommended hair care regimen between appointments that may include but is not limited to:

      • Using the recommended professional shampoo, conditioners, and styling products.
      • Only use recommended hair brushes on extensions. Brush hair multiple times a day to detangle, holding the base of extensions and work your way up to the top of the weft
      • Blow dry the top of extension wefts so they do not hold moisture for an extended period of time. Never go to bed with wet hair.
      • Oil the mids to ends of extension hair daily.
      • Use a silk or satin pillowcase and sleep in a low braid or ponytail.

      Be cautious of the following:

      • Avoid getting sunscreen in the hair and only use sunscreens that are free of
      • Avobenzone and Oxybenzone that can cause hair to turn a brassy pink/peach color.
      • Avoid hard water, salt water, and chlorine- these can strip both natural hair and extension hair of color. Use a water softener if necessary, and although it is not recommended, if you do choose to go swimming, saturate your hair in conditioner and wear it in a braid.
      • Avoid high temperature settings on hot tools and blow dryers and replace styling tools every 2-5 years.

      Recommended maintenance appointments:

      •  Extension maintenance appointments are every 6-8 weeks, anything beyond 8 weeks is not recommended as it increases the risk of the extension hair matting and can cause stress to the natural hair. Please keep in mind that the proper maintenance schedule can vary for each client depending on hair type and not everyone will be able to go beyond 6 weeks.
      • Recommended color maintenance appointments will vary for each client and will be determined by the stylist. Please keep in mind that some level of maintenance is still required for even the lowest maintenance colors due to the fact that all artificial hair color fades over time and at different rates due to porosity, haircare, and hair type. 
      • In the event that an extension bead pops loose or stitching comes undone before the regular 6 week appointment time frame, the stylist may offer to inspect and restitch when possible. This is a rare occurrence and if it is determined to be a recurring issue due to improper care the stylist may recommend removal of the extensions.

    • Cancelation & Payment Policy 
    • I understand emergencies sometimes come up and you may have to cancel or reschedule your appointment. I kindly request the respect of both my time and the time of others by providing a minimum 24 hour notice of any changes to your service or appointment time. Failure to provide sufficient notice may result in a late cancellation fee of 50% of the scheduled service.

      If for any reason you are expecting to be late by 10 minutes or more to your scheduled appointment, please contact the salon ASAP. Anything beyond 15 minutes may be considered a no-show and subject to a fee of 100% of the scheduled service. Accommodating late guests is often impossible without disrupting other appointments scheduled that day. Please be aware that in the unfortunate event you are 15+ minutes late, I cannot guarantee that your services will be performed that day or performed in full. Tardiness may require rescheduling your appointment for another day and subject to a no-show fee. On the occasion that an abbreviated service may be performed, they will still incur the full charge of the scheduled services.

      In the rare event that I, the stylist, must cancel the appointment at the last minute due to an emergency or illness, every effort will be made to contact the client as soon as possible. I will offer to reschedule the appointment at the earliest available time that is convenient for both the client and the stylist. In this situation, no late cancellation fee will be charged to the client.

      Payment is due promptly at the time of the appointment. Payment methods accepted are Cash, Card, and Apple pay.
