Thank you for considering Howard Tax Prep LLC to assist you with your payroll needs. In order to start direct deposit payroll for your employees, each employee must have:

Valid Identification.
Federal W4 form.
State W4 form.
Completed TYPED direct deposit form.
Employee information sheet with name, email address, and mailing address.
Date of hire.
Previous cash payments made. If no cash payments made, this is not necessary.

Please note, these forms can be provided to you at no cost, should you choose our company to provide your payroll services.
