Psychological Services
As part of providing psychological services to you, your psychologist will need to collect and record personal information from you that is relevant to your current situation and concerns. This information is necessary to inform the assessment and treatment service in order to accurately assist you. Your informed consent is obtained before any treatment is conducted, and you may withdraw from treatment and therapy at any time.
Your Information
Your information is gathered as part of the assessment, diagnosis and treatment of your condition and difficulties, and is only seen by the psychologist. The information is retained in order to document what happens during consultations and enables the psychologist to provide a relevant and informed service to best assist you. At any stage you as the client are entitled to access the information about you kept on file, unless the relevant legislation provides otherwise. Confidentiality and Exchanging Information
All personal information gathered by the psychologist during the provision of the psychological service will remain secure and confidential except in circumstances where:
1. your information is subpoenaed (officially requested) by a court, or
2. Failure to disclose the information would place you or another person at serious risk of harm and/or danger; or
3. Your prior approval has been obtained:
a) to provide a written report to another person, professional or agency; or
b) to discuss the material with another person (such as an employer or family member); or
c) if disclosure is otherwise required or authorised by law
If you provide a referral from a General Practitioner (GP) or psychiatrist, we will contact them to confirm acceptance of the referral, to provide regular treatment reports, and to provide a letter upon the completion of your treatment, even if you discontinue with therapy.
You may be seeking to have your treatment sessions covered by a third party including, but not limited to, WorkSafe Victoria, the Traffic Accident Commission (TAC), or the National Disability Insurance Scheme (NDIS). In these instances, we will contact the listed organisations to confirm your eligibility for coverage, including any limitations that may apply. Any request from these organisations for treatment reports and updates will be discussed with you before detailed information is provided.

Cancellation Policy
You will receive an email/SMS reminder 48 hours prior to your scheduled appointment. If you need to cancel or reschedule your appointment, you are required to provide at least 24 hours-notice. This is to allow other clients on the waitlist enough time to book this appointment time. A cancellation fee (100% of session fee) will apply if 24-hours notice is not provided, however, this fee may be waived for medical reasons and if a medical certificate is provided.
