Inclusive Swimming Programme For Students with Special needs

General

1. Participants signed up for the lessons at their own risks.

2. HomeTeamNS, Torpedo Swim Team Pte Ltd and / or and their appointed representatives reserves the rights to cancel or postpone the lessons due to any unforeseen circumstances without giving prior notices.

3. Participants are not allowed to engage Torpedo Swim Team Pte Ltd's swimming instructors for private lessons outside of the clubhouse. 4.HomeTeamNS reserves the rights to amend the terms and conditions from time to time where necessary and appropriate without prior notice.

 

Cancellation/Replacement of Classes

1. Land drills will be conducted in sheltered areas if inclement weather/ bad environmental conditions (i.e. Haze with 3 hours PSI reading at 151 to 200 In such conditions, the pool does not permit the conduct of swimming lessons. Lessons affected will not be replaced nor refunded.

2. There shall be no replacement of classes except for cases where: a. lesson is cancelled due to the absence of Instructors; b. Participant is absent for medical reasons, in such cases, participants must produce a Medical Certificate.

 

Payment of Fees

1. Each new term starts every quarter in January, April, July, and October. All Terms consists of 12 lessons.

2.Payment shall be made on a quarterly basis. Should a participant join mid-term; the "per lesson' fee applies till the end of the quarter.

3. Lesson fees must be made in full before the commencement of class via the Customer Service Counter. A receipt will be issued and the participant produces the receipt to the Lesson Coordinator for verification.

4. For child aged 5 to 21 years old, the Child must have his / her own Family Membership card to enjoy member's

5. Family membership for child applicant is only for age 5 and above up to the age of 21 years old. For child below 5 years old, the child can tag on to his / her Parent membership card to enjoy member's rates.

6. Fees must be paid one month in advance in full before the commencement of the term to reserve a place in the preferred class.

7. All fees paid are strictly non-refundable and non-transferable.

8. All fees include GST.

9.There is no entry fee to the swimming pool for the participant. Each participant will be issued with an additional swimming pool pass for his / her accompanying parent /guardian. The pass will be issued by and to be returned to the Pool Coordinator on the day of the swimming lesson.

 

Auto Renewal*

1.The student may continue the lesson in subsequent term without having to submit a new Registration Form.

2. The registration shall be renewed based on the same Terms and Conditions herewith.

3. The auto renewal will cease when:

a. there is non-payment of fees within 10 days when the new term starts, or/and

b. there are changes in the particulars of the student, or/and

c. there is change in the membership type affecting the amount of fees payable, or/and

d. there is a break in between the lesson terms

4. I agree that fees are non-refundable unless the activity is cancelled by HomeTeamNS.

5. Course confirmation will be notified by Torpedo Swim School.

6. I acknowledge that Parent / Guardian consent is required for applicants under 21 years old.

7. I hereby declare the participant to be in good physical and mental health to take part in this course.

PERSONAL DATA PROTECTION ACT (PDPA) DECLARATION:

HomeTeamNS recognizes its obligation under the Personal Data Protection Act 2012 (PDPA) which include the collection, use and disclose of personal data (information provided above) which you have given consent to by the submission of this registration form. Your personal data belonging to you and your family may be used and disclosed to HomeTeamNS's affiliated organisations, third party service providers (activity partners etc for the following purposes:

a) Register and contact you about your attendance at the activity;

b) Carry out administrative, planning activities and to facilitate your requirements for the activity;

c) Plan and develop future activities and opportunities;

d) Compilation and analysis of statistics relevant to the course/activity;

e) Providing marketing material that you have agreed to receive.

As a valued HomeTeamNS member/customer, you can expect HomeTeamNS and its affiliated organizations, service providers and programme partners to communicate to you via channels such as electronic direct mailers (Email), Short messaging service (SMS), direct mailers (Postal Mail), whatsapp and phone calls (Voice).
 
By attending the activity stated above, you consent to the use of photograph, voice, likeness, and image in any broadcast of this event and in subsequent productions drawn from video or audio recordings of this event. The photographs and recordings may be published or broadcasted in the official HomeTeamNS and HomeTeamNS affiliates' publications and in publicity materials, including the HomeTeamNS and HomeTeamNS affiliates' websites and social media. All recordings, articles, quotes, photographs, films, CCTV footage, audio or video and/or any reproductions of same in any form, are the property of HomeTeamNS.  HomeTeamNS and its affiliates, employees, representatives and agents are released from any and all claims, demands, costs and liability that may arise from the use of any of the described above, arising out of being interviewed, recorded, photograph, videotaped or filmed.
**It is the responsibility of event participants to remove themselves from group and candid shots/recordings that the photographers/videographers intend to capture, if they do not wish to have their photos/audio-visual recordings taken at the event.**
 
 
By providing the particulars of my emergency contact above, I confirm that I have obtained the consent from the individual named to provide their personal data to HomeTeamNS for HomeTeamNS to contact him/her in case of emergency (the “Purpose”). I agree to notify HomeTeamNS if the individual withdraws consent to the use and/or disclosure of his/her personal data for the Purpose, and to assist HomeTeamNS with all access requests and complaints which may be received from the individual regarding the use and/or disclosure of his/her personal data.
