This emcompasses any and all services Devon Curly Girl LLC offers. I also recognize that there are no guaranteed results and that independent results are dependent upon age, skin, hair, condition, lifestyle and other factors.
I understand that advised items such as supplements and hair and body products are only recommendations. I understand that i may request a patch test at anytime for any products before service and by signing this i am waiving that option and take responsibility.
I have read and fully understand this agreement and all the information detailed above. All my questions have been answered to my satisfaction and I consent to the terms of this agreement. I understand and assume any risk(s) associated with any and all services, recommendations, supplements and/or advice given by Devon Curly Girl LLC and therefore I agree not to hold Devon Curly Girl LLC responsible or liable in any way.
