Please enter the requesting external Dr details - include the name and which surgery or organisation it comes from eg Dr John Smith at the Smith Street Surgery or Dr Joanne Smithson at the London Clinic-can be a GP or consultant. Also include the telephone number and email address for results to be sent to. Please ensure you bring the request form with you to your appointment.
Results for self-requests will be sent to you by email. We can send the results to your doctor if you include the name and email below. If you would like to discuss your results with a JDoctor at JDoc365, you can book an appointment through reception at the usual fee rate. If you do not want to share the results, please write NA below.
