Driver’s responsibility

Under the Road Traffic Act 1988 (RTA) the driver is responsible for ensuring that the vehicle
is in a roadworthy condition. This means on any occasion you take a vehicle onto the road you are:

• responsible for the vehicle.
• responsible for checking the vehicle for any defects before commencing any journey.
• breaking the law if you take an unroadworthy vehicle onto the public highway.
• liable for prosecution if there is a fault which could result in a criminal record.
• Drivers of all vehicles must check their vehicles before first use of the day.
• Drivers must never drive a vehicle knowing that it is in an unroadworthy condition.

Law enforcement
Traffic law enforcement is undertaken by the Police or officers from DVSA (Driver and Vehicle Standards Agency). Local Authorities also have powers to prosecute for motor offences such as parking, route violation and zone restrictions.

This document will be reviewed every 3 months or if any significant criteria change

SITE RULES AND TRAFFIC MOVEMENTS

Fastsource, The Old Coal Yard, Caldervale Road Wakefield, WF1 5PH (012924 200005)

Site rules are there to ensure that all employees, contractors, and visitors can use the site and leave without being injured. All contractors and visitors must sign the visitor's book in the weighbridge office on entering the site.

The site rules are as follows:

1) On entry ensure all loads are securely netted/sheeted.

2)Be alert to the health and safety of all, and wear the correct safety clothing, including high visibility vest, pants, jackets, safety footwear with steel toe caps and midsole protection, gloves, safety helmet and ear protection SNR27 or up to SNR30

3) Ear protection must be worn by all contractors and visitors entering Fastsource yard when crushing and screening are in process. If any drivers dis-mount their vehicle, then ear protection must be worn. Ear protection must be SNR 27 or up to SNR30. If anytime drivers, contractors or visitors don't have ear protection then Fastsource have ear protection on site in the weighbridge office. You must sign confirming receipt of ear plugs and understand how to fit correctly.

4) Mobile Phones must not be used whilst operating or driving vehicles on this site.

5) Do not smoke, eat, or drink within the waste processing areas. A hygiene regime is maintained for your safety.

6) Stop on the weighbridge and report to the site weighbridge office.

7) Ensure Waste Transfer Notes and other relevant paperwork is completed for your aggregate materials and handed to site staff before unloading aggregate materials.

8) Weigh your vehicles both before and after depositing your materials.

9) Obey all instructions given by site control personnel.

10) Travel only on recognised routing systems and obey all traffic signs and speed limits 5 M.P.H., be aware of speed bumps and maneuvering plant machinery in the treatment station area.

11) Discharge loads only in the correct stockpile and in the manner directed by the code of practice and permit to work.

12) Ensure isolation of the loading zone, ensure the vicinity is clear of non-PPE-protected personnel during all loading and unloaded operations.

13) Sorting over and removal of items from this site will be regarded as theft and offenders will be dealt with as such.

14) Inform site staff of any accident/spillage or possible hazardous waste contamination in your load.

15) Good conduct is expected of all users/visitors, threatening or abusive behavior towards staff will not be tolerated.

16) Anyone failing to comply with these site rules will be stopped from using the site until they are able to fully meet the requirements.

17) Before leaving site check your vehicle is safe for the highway, no loose load content, no punctures, no stones trapped between tandem wheels.

18) Ensure all vehicles travel no more than 10 M.P.H when traveling past the Esseco chemical plant and JHM garage.

Please note, health and safety is everyone's responsibility if at any time you have an accident to your person or vehicle whilst using our site No matter how minor you must report it immediately to the site manager/staff and fill in the accident book located in the weighbridge.

Ensure that your work area is tidy and free from any rubbish

Waste disposal and recycling compounds are provided on siteSpot checks will be completed by the HSEQ manager.

Rubbish is a hazard and must be processed.

Fire procedure

Raise the alarm at activation points.Locate an escape route.

Make your way calmly to the designated meeting point (side car park) and stay there.

If the area contains smoke get down as low as possible and proceed as quickly as possible along your exit route.

All drivers of road going vehicles must ensure the wheel nuts on their vehicle are secure this is done by carrying out the following procedure.

Checking tyres and wheel nuts condition & security

The Driver of a vehicle is responsible for the safety and checking of a vehicle before use, an important part of the daily checks carried out by a driver are the condition and security of the tyres and wheels.

In order to assist drivers in the daily checks of wheel nuts

Drivers must visually check their vehicle tyres and wheel nuts every day this should be carried out whilst undertaking their daily vehicle checks.

Tyres should be checked for damage and wear, tread wear markers are fitted to all tyres (see illustration) these are a guide to show when the tyres tread is below the legal limit, if the wear indicator is level with the tread the tyre requires re-cutting or replacement.

Wheel nuts should be checked visually for signs of wear if a nut is loose there may be discolouring on the wheel around the nut (i.e., rust or water marks), If you have any doubt regarding the nuts security then the torque wrench should be used to check the nut and tighten if required.

Removal of wheels from vehicles

Following a puncture or tyre change where the wheel has been removed from the vehicle the wheel nuts will require a re-torque, a re-torque tab issued by the tyre company or trained staff will be left in the vehicle or site weighbridge detailing which wheel has been changed, the ticket should then be returned to your site manager office and the re-torque record form (FS 001) completed. Wheels requiring re-torque must be done the following morning before vehicle leaves site.

In addition to individual wheel nut re-torque, drivers must ensure they carry out 3rd weekly re-torque with their torque buddies. A full check of their vehicles wheel nuts. Both buddies must recorded and completed on the 3rd weekly check sheet (FS 002)

Drivers using other vehicles which may have been stood for long periods must not presume the wheel nuts have been checked or re-torqued, these vehicles should be checked before use.

Vehicles returning from service after either a service or repair may have had wheels removed, if this is the case the collecting driver should find a re torque ticket fastened to either the vehicles steering wheel or driver keys, these vehicles will require a full re-torque check of the highlighted wheels and wheel nuts before use.

Punctures and wheel changes away from Fastsource yard

If you have a puncture away from your depot and the wheel has been removed, on your return to the depot you must re-torque the wheel and complete the re-torque ticket issued by the tyre fitter or trained staff, the ticket should then be returned to your site manager office and the re-torque record form (FS 001) completed.

If you do not receive a re-torque ticket from the tyre fitter or trained staff who has repaired or replaced the tyre, you must make a note of the vehicle registration , wheel position, name of tyre fitter and the time and date, then upon returning to the depot you MUST re-torque the wheel and fill in the re torque recording sheet (FS 001) whilst also informing your site manager that you did not receive a re torque ticket, your site manager will then raise the issue with the with trye company or trained staff as a matter of urgency.

Re-torque of wheels, using the torque wrench

The torque wrench on Fastsource site is located inside blue container nearer workshop. The torque wrench must always be wound off after use, if you find a torque wrench not wound off, please inform your site manager who will then try and find the last user so that they can be re- trained in the use of the torque wrench. The torque wrench must then be wound off before resetting the correct torque at 640NM. Re-torque wheel nuts can be carried out individually or as best practice in a two-person team.

To adjust the torque wrench setting turn clockwise to torque setting 640NM

Where fitted all wheel nut markers must be removed before the wheel nuts can be torqued.

Re-aligning wheel nut markers

After completing a re-torque or check of the wheel nuts, all markers where fitted must be re fitted and lined up to the correct position, markers should face point to point and be fitted to all wheel nuts, If you see a marker missing inform your site manager so he can have it replaced as soon as possible.

 Wheel nut Alignment marker, showing movement this indicates a wheel nut that may have moved. The wheel nuts on this wheel may be loose and requires checking with a torque wrench.

Paperwork and Record Keeping

Driver's 3rd weekly checks FS002 (Example see below)

All drivers must visually check their vehicle wheel nuts daily as part of the pre start vehicle checks system, in addition on a 3rd weekly basis drivers must carry out a full wheel check using the torque wrench, each nut on every wheel must be checked and then recorded on the 3rd weekly re-torque record form FS 002, details must include vehicle identification, drivers name, day wheel nuts checked, the driver's signature and torque buddies signature.

If for reasons beyond the drivers control or following agreement with a site manager these 3rd weekly wheel nut checks are not completed by the end of the working week the driver must inform his site manager, so that action can be taken to have the wheel nuts checked.

Re-torque FS 001 and driver checks record form FS 002 are in the site office.

Job Sheets

Following the removal of a wheel by the tyre company fitter or trained staff an electronic job sheet will be created this will detail the work carried out and tyres replaced fitted, the driver or responsible person must sign the electronic pad or sheet indicating the work has been carried out.

Once signed and sent the job sheet is emailed to Fastsource and for record keeping.

Re-Torque tickets

Following the removal of a wheel by the tyre company or trained staff, a re-torque ticket will be completed an example is illustrated bellow, the tyre fitter will complete the relevant sections of a re-torque ticket and either hand the ticket to the driver or leave the ticket prominently displayed in or on the vehicle.

On the front Section (1)

this section is completed by the tyre fitter or trained staff and shows the date/time, mileage and the torque setting the wheel has been tightened to. The tyre fitter or trained staff will sign and print their name confirming they have carried out the initial torquing of the wheel nuts.

On the rear

The tyre fitter or trained staff will complete the section wheel position details using an X to indicate the position of the removed wheel in cases of more than one wheel removed a second X will mark the position as illustrated.

The tyre fitter or trained staff will also complete the attached vehicle id the vehicles registration number.

Completing the wheel re-torque

Once a driver has completed the wheel nut re-torque, using the torque wrench as per instructions above, the re- torque ticket issued by the tyre fitter or trained staff must be completed and signed to confirm the re-torque has been carried out. The re-torque ticket should be signed by the person carrying out the re-torque on the front of the ticket, section 2 the re-torque ticket should then be handed in to the site manager for processing.

As part of Fastsource safety check system the additional re-torque record form FS 001 must also be completed please see examples below (part of the form may have already been completed by office staff following receipt of the job sheet) the remaining parts must be completed by the person carrying out the re-torque. Details must include (vehicle registration, position of wheels re-torqued, date, and driver's signature)

Old incomplete wheel re-torque tickets

if at any time you find a wheel re-torque ticket that has not been signed off as completed, and is several days old, then you must presume the wheel nuts to be loose and inform the site manager immediately handing in the incomplete re-torque ticket, so that the safety breach can be investigated. The corresponding vehicle MUST NOT be used until a full re-torque has been undertaken on all the vehicles wheel nuts. The driver must re-torque all the wheels on the vehicle and record the wheel re-torque on the vehicle weekly re-torque sheet (FS 001) the incomplete re-torque ticket must also be completed and signed as re-torqued and the re-torque record form FS 001 completed.

Wheels removed and no re-torque ticket issued.

Whilst this should not happen, we have in the past had incidents, examples are if a tyre is changed away from base by a sub contractor and no ticket is issued or when the tyre fitter has run out of re-torque tags, to prevent a safety risk of a wheel becoming loose the following must be followed.

Drivers

If a wheel is removed from your vehicle or you notice a possible new wheel fitted and you have not received a re-torque ticket, you must inform site manager immediately so it can be chased up.

Office staff will on receipt of a job sheet showing a wheel removed, notify the driver of the vehicle so that a re-torque can be carried out the driver carrying out the re-torque will be required to sign and date the job sheet to confirm the wheel has been re- torqued.

Wheel nut sockets over time will become loose with wear; Drivers are reminded to notify the site manager if a socket becomes worn for a replacement.

Digital Tachograph - Driver Card

Tachographs are essential record information ensuring; driving time, speed, distance along with ensuring drivers are adhering to the rules on driving hours.

Current legislation requires you to have your cards downloaded at a minimum of every 28 days.

It is our company policy that tacho driver cards MUST be downloaded every day after your working shift at Fastsource Ltd

Failure to comply, the company reserves the right to retain monies owed until the information has beed supplied/downloaded.

 AGENCY WITNESS AGREEMENT FORM 

Notwithstanding the conditions contained in our Employment Terms of Business, our Insurers may, either during your working time at Fastsource, employment, or after leaving the company, require you to assist in defending or pursuing a possible claim following any accident. 

 This may require you to provide additional information, statements and possibly appear as a witness in court (upon which reasonable expenses will be allowable).

 Should this be required, you hereby agree to provide your assistance wherever possible and within a reasonable time.

Specific Site Hazards!

• Working at Height: Don’t climb steep piles, put in a walkway to your machine (excavator, shovel, crusher/screener). Use the steps provided, using three points of contact.

• Vehicle Movement & Plant: We will always tell drivers when the plant is in operation, and where to proceed to the correct stock piles, for tipping and loading.

• Asbestos: We are asbestos trained in the yard. If you do see any alert a member of the management team, who will attend to it.

• Lifting Operations: All our machines have lifting certs.

• Hazardous Contaminated Loads: This would include Tyres, Gas Bottles, Wet Plaster Board, and Lithium Batteries.

• Fuelling of Machines & Wagons: You must wear gloves, and refrain from smoking whilst fuelling up.

Please make use of this facility when approved by either the site manager or site supervisor.Spot checks will be completed by the HSEQ manager.

Rubbish is a hazard, contractors not cleaning up during and after their work will be issued with warnings.

Any cleaning costs incurred by Fastsource Ltd after agency driver use will be passed on to the offending Agency

Your Responsibilities

As employees you have the responsibility to ensure that your conduct and daily tasks are maintained safely and don’t cause harm to yourselves or others.

Things to consider:

Helping their employees create a safe working environment and abide by the regulations set in the workplace.

Following the procedures put in place to stop accidents and overexposure which causes harm.

Wearing the correct PPE, such as eye protection and noise protection.

Ensuring the PPE is stored correctly and returned to its correct storage facilities

Correctly reporting any accidents/spillages/breakages of equipment etc

Using any cleaning and showering facilities that are provided by employers.

Substitution: Stop the activity (or stop using certain raw materials) and replace it with a process that poses less risk

Technical measures: Structural or procedural changes that better protect the employee (e.g., protective walls, extraction of fine dust)

Organisational measures: Changes in work procedures that ensure that employees are exposed to the hazard only when absolutely necessary (e.g., access restrictions, increased maintenance intervals, limited working time)

Personal measures: Provision of personal protective equipment (PPE) such as respiratory protection, safety goggles, or fall protection
