In addition to the application above, you also need to submit proof of certification and proof of payment. Please note that cancelled checks or invoices that do not show proof of payment are not an acceptable form of documentation. Your certifier may provide DATCP with this supporting documentation, but this needs to be arranged between you and your certifier.
