Ohio State USBC Scholarship Form

The Ohio State USBC Scholarship is awarded annually, recognizing exemplary qualities to 1 male and 1 female high school student. The scholarship winners will receive $3,000.00 to be placed in their SMART accounts. To be eligible, candidates must be a current USBC member in good standing and maintain at least a 2.5 GPA or equivalent. Fill In all information completely. Use a blank sheet of paper to continue all answers and number them to correspond with the question number (i.e.: Section 1, Section 2, etc.)
