Payment Options

I would like to purchase the Dancer Start Up Kit. This kit includes Black Tap shoes, Ballet Pink Ballet shoes, Ballet Pink Tights, Leo, and a fun surprise for $95.00.

I understand that recital will be held in May for Pre k Classes and above. Costumes will be picked out and pricing will be available by November 1st. All costume fees will need to be paid by January 1st. (Dec. 1st 40.00, January 1st 35.00) $75.00 total per class. This is non refundable. This will ensure that I can get orders in and costumes here for the May recital. Recital will be Mother's Day weekend (dress rehearsal Friday 5pm-7pm and Recital Saturday time TBD). Photos will be taken at dress rehearsal (Friday) for additional charge.

Photos are taken for promotional purposes, including but not limited to posting a copy of photos on Tapping Toes School of Dance Website and Facebook page. If you will allow your child’s photo to be published please mark the yes box if not please mark the no box. There will be a digital copy of this form attached to your file in the computer and a hard copy will be kept on file for the current year. This form must be filled out yearly with the registration paperwork.

I understand that Dancers may make-up any lesson missed by talking with their teacher and making arrangements during the same month a lesson is missed. I also understand that all fees paid are nonrefundable and nontransferable. The returned check/declined card fee is $15 for first offence and second offence student will be dropped from class. Should this provision have to be enforced by legal means, the undersigned person(s) is responsible for payment, as liquidated damages, the costs of collection, plus interest at the legal rate and reasonable attorney’s fees as determined by the Court or 15% of the amount collected failing such determination. Registration fee of $25 for the first student. $10 for each additional student in the family. Payments are due the first, late fee will be applied on any payment after the 10th. Payment is due regardless if you attend all classes that month or not. If you choose to drop class, there is a 30 day written notice required.

Indicated in the space below are any health problems or conditions of which the studio should be aware (such as heart, back, medical, allergy, muscular, pregnancy, diabetes, epilepsy, chemical or neurological condition, special medication, knee/kidney/shoulder problems, etc.). I understand that risk of injury is inherent in any physical activity and I, on behalf of myself and my child, knowingly and voluntarily accept that risk. I, the undersigned, for myself, my heirs, administrators, and executors, hereby waive and release Abigail McGee individually and Tapping Toes School of Dance, Inc. and its staff from any and all claims or damages of any kind arising out of my child’s participation in the exercise and/or dance program of Tapping Toes School of Dance, Inc. I further certify that the aforementioned student is in proper physical condition to participate in the exercise/dance program and that he/she has been examined by a licensed physician and found to be in proper physical condition to participate in said program. I, the undersigned, do hereby authorize Abigail McGee or her designated agents (being teachers or administrators employed by Tapping Toes School of Dance, Inc.) to obtain medical treatment for my said child in emergency situations where I cannot be reached in time to authorize the treating physician to provide such emergency medical services. I understand that I am responsible for any medical expenses and that the absence of health insurance does not make Tapping Toes School of Dance, Inc. responsible for payment of medical expenses. This authority includes the power to authorize any and all treatment deemed necessary under the circumstances by a licensed physician. This power is in essence a power of attorney and shall remain in effect for one year from the date signed below. By providing your e-mail and cell number you agree to electronic communication. Tapping Toes School of Dance uses the remind app. I understand that Tapping Toes School of Dance is taking precautions to help slow the spread of COVID-19. I also understand that bringing my child to dance could raise their chances of contracting COVID-19.
