Roster should list every player who will participate for your team during the event. Indicate any "guest players" by denotation of "guest player" next to player date of birth. IF typing a roster, be sure to list players by last name, first name, date of birth, jersey # (IE: Maradona, Diego, 2/1/62, #10).

I verify that every player listed on the roster is of the correct age for the age division they are participating in this event. Each player's listed birthdate on the team roster is correct. Only players listed on the team roster will participate. I verify each player participating is duly registered in the 2022-2023 or 2023-2024 seasonal year, certified as registered and insured by my club.

I verify that our team has in possession a player pass card which serves as a proof of birth and proof of registration for every player participating in this year's event. The player cards/player pass cards are from the 2022-2023 or 2023-2024 seasonal year.

MEDICAL RELEASE VERIFICATION

I verify that our team has in possession a medical release/waiver document (either USYS or US Club or USSSA). We will keep these on hand at all times during our team's participation so that if needed they can be produced at the fields.
