This form is for guests who have seen me within the last 6 months. If you haven't seen me in more than 6 months, please fill out a new guest appointment request at www.azjorhairdesigns.com/new guest so I can get caught up on your history. I will be in touch within 24 hours of your request during my work week which is Tuesday-Friday. All requests submitted Saturday-Monday will be responded to on Tuesday. By Completing this form, I am submitting an appointment request and price quote inquiry. In reponse, I will receive an email with an appointment time and date option and a general price quote. Generally, Natalie will be able to get you in to the salon within the next 2-3 weeks, but guests with limited availability may have to wait up to 4-6 weeeks to be seen. I understand that Natalie has a strict 24 hour cancellation policy and if I do cancel my appointment with less than 24 notice, I understand a fee representing 25% for each service missed will be added to my profile and must be paid before I book another appointment. For example, if you where booked for a color and haircut service, the cancellation penalty would be 25% on the color and 25% on the haircut.
