  • PLEASE READ THE FOLLOWING:

Please complete all the information on the following pages. Missing information may delay or result in no contact in the foster/adoption process. Completion of this application form does not guarantee that ELLD will place one of our dogs in your care. Completion of this questionnaire authorises that Alexis Aitchison @ ELLD can verify any information contained herein, including verification of medical records of any present or prior pets treated by the veterinarians listed. Completion of this adoption process is an indication that you have read, understand and agree the information contained within this application is accurate and not misleading in any way. Edinburgh and Lothian Lost Dogs - Privacy Policy

  • We are required by law to explain how we store, use and process your data. Our Edinburgh & Lothian Lost Dogs - Rescue privacy policy can be found on the link above. Please tick the box below to show you have read, understand and agree.
  • Please give careful consideration to adopting a dog. Dogs are not short-term commitments. Make sure your lifestyle allows the time, patience, training, and expense this dog will need over the years. All dogs will be spayed/neutered, vaccinated and microchipped prior to adoption.

  •  -
  • Phone Type*
  • Pets

  • Have you had other animals as pets in the past?*
  • Home

  • Do you rent or own this property?*
  • If you rent, you must provide proof you are allowed pets, this can be a written letter from your landlord or a copy of the section of your lease stating that you are allowed to have dogs and showing any limitations. Or you can provide us with your landlord's contact information.

  • Are you planning to move in the near future?*
  • Work & Lifestyle

  • The Dog

  • Based on the description in the dog's bio, do you feel pretty confident that you will likely suit and manage their needs and their lifestyle?*

  • Have you discussed this adoption with all members of your household?*
  • We will be carrying out a home check if the applicant is successful in this part of the process and speaking to all household residents to confirm everyone is aware of this application process and wants to have a dog added to the family. Please do not submit this application if you are "trying to talk someone into" adopting this dog or think "once they meet the dog, they will want one" or "I'm getting this dog as a present for them." In those scenarios, you are welcome to email us to discuss the dog, but we would like all adults in the home to be involved in that email discussion.

  • What activity level do you prefer?

  • Please check all that apply. I want to adopt this dog...*
  • Please check all that apply. My adopted dog will...

  • Please take time to think, be honest with yourself and us when answering this question. What are you absolutely NOT willing to deal with at the time of foster/adoption or at any time during the settling in period and the life of the dog?*

  • Do you plan to get pet health insurance?*

  • Are you in contact with any other rescues or shelters about a specific animal?*
  • Are you going to meet another animal?*
  • If you are successful in this part of the application process you will receive an email and we will move forward with a home check. If successful after the home check we will move forward.

Adoption Contracts will be signed with the dog having life time rescue back up which means even after adoption, for any reason that circumstances change where the dog is not receiving the attention and care needed, or that the dog cannot be looked after any longer by the adopter then they will be returned to Alexis Aitchison @ ELLD. The rescue back up is provided to ensure the dog has a life line for life and also ensures the adopters have that lifeline too.

Thank you for taking the time to fill out the application form. And I thank you for your patience in advance while you await on a reply.

This is solely run by myself as a volunteer and takes a tremendous amount of time to co-ordinate everything behind scenes with the hands on behaviour work with the dogs, the travelling to kennels, the rehab, the daily husbandry with the sanctuary dogs and general day to day admin.
The applications that are completed fully and are a match to the lifestyle and needs of the dog will be prioritised. The most important thing is matching the dog to the right home and prioritising the time correctly to do this.
    Thank You

    Alexis @ ELLD
