I acknowledge that Posh+Polish Ltd. and its employees to the best of its ability provide a safe and clean work service environment and I confirm that Posh+Polish Ltd. and its employees cannot be held liable for any mental or bodily harm as a result of my choice to willingly take part in its services or to be on the premises.I acknowledge that products may contain FDA approved chemicals and I permit the usage of the necessary products as part of my service treatment and that Posh+Polish Ltd. and its employees cannot be held liable should any adverse affects occur.I affirm that I do not have any cold, flu or Covid-19 like symptoms including but not limited to fever, chills or cough and if I did I understand that due to the personal nature of the services I am required to cancel and furthermore reschedule only once my symptoms are gone.I acknowledge that I have read and understood this document in its entirety and although I will submit this waiver only once, I intend to be bound and remain bound to the terms herein for as long as I continue to receive services from Posh+Polish Ltd. and its employees.By signing below I hereby release and discharge Posh+Polish Ltd. and its employees from any and all liability in connection to its services, treatment and premises and further understand and agree that I hold Posh+Polish Ltd. and its employees harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation from matters that arise in any way in connection to the premises, treatment and services received from Posh+Polish Ltd.
