This is a form for the students/children who would like to receive a FREE haircut by Phil Johnson from Do or Dye Hair Salon (Concord) and Jong Pak from California Haircuts (Lafayette).

You will automatically receive a backpack at your appointment. If you don't need one, please indicate that on the form.
