  • INTRODUCTION

    To apply for Legal Aid for Family Mediation you must provide all applicable evidence and confirm all relevant details for the calendar month up to and including the day of your appointment and sign to confirm that the details are correct.

    To assist you in doing that, we have created this form.

    There are three parts to the form, please complete all three.

    The form should take around 5 to 10 minutes to complete.

  • Was your surname different when you were born?*

  • Do you receive Universal Credit?*
  • In the last month, have you been employed, self-employed or worked in an occupation?*
  • If yes, are you currently employed, have you been employed, self-employed or in an occupation?*
  • Do you have a partner with whom you live?*
  • If you are living with a new partner, you must also provide all the evidence that applies to them as explained in Section 3 below.
  • Do you own any property?*
  • Do you live in the property that you own?*
  • How is the property owned?*
  • Is there a mortgage outstanding on the property that you own?*
  • Is your dispute about this property?*
  • Do you have more than £8,000 in savings?*
  • Do you have any savings?*
  • How are your savings held?*
  • Section 2: Your Signature

  • Your Declaration

    • As far as I know, all the information I have given is true and I have not withheld any relevant information. I understand that if I knowingly give false information or withhold relevant information I could be prosecuted.
    • I understand that I must tell you immediately if there are any changes in my or my partner's financial circumstances.
    • I agree to the Legal Aid Agency checking these facts with others such as the Department of Work and Pensions and the HM Revenue and Customs and I authorise those people/organisations to provide the information they are asked for.

    Your signature will be used to sign the Legal Aid application form, which can be found [HERE].

  • Section 3: Your Evidence

  • Finally, you must provide evidence to confirm your circumstances, the section below will provide information about the evidence required. You can submit your evidence using the Upload Evidence section below.
  • What (if any) income have you received in the last month?*
  • Universal Credit

    If you receive Universal Credit you can send your Legal Aid evidence by providing a copy of the most recent monthly statement you have received from your Online Journal.  

    You must provide the WHOLE statement. Screenshots of part of the statement will not be sufficient and will delay your assessment.

    You can see some examples of statements the Legal Aid Agency will accept [HERE].

    You can get the statement by:

    • Logging into your online journal
    • Accessing payments
    • Select the most recent payment received 
    • Select print in the top right
    • Select "print to pdf" from the printer options
    • Screenshot the whole statement, from the top to the bottom.

    Then upload the screenshots below.

  • ESA / JSA / IS / GPC / Other Legacy Benefits

    Please upload a copy of a Proof of Benefit letter or an award letter received in the last six months, that confirms:

    • your name,
    • your address,
    • the amount that you receive.

    If you need to request a new letter, the following may be helpful: https://www.gov.uk/contact-jobcentre-plus/existing-benefit-claims

    You must upload all of the pages of your award letter, even if they are blank.

  • Other Income

    If you have not received any of the other forms of income above then please contact us at: admin@paxmediation.co.uk to confirm:

    • The income you have received in the last month (from all sources)

    AND

    Whether you have paid any of the following:

    • Rent / Mortgage / Board
    • Childcare (due to work)
    • Child maintenance

    Then we will contact you to confirm the evidence that you will need to provide.

    Please ensure that you have uploaded the WHOLE of your Universal Credit statement

    The MUST include:

    • Your Name and Address.
    • The date of your Universal Credit payment.
    • EVERY part of the statment from the very top, to the very bottom.

    If there is ANY part missing from your statement it will delay your Legal Aid assessment and appointment. 

  • 🚨 PROMPT 🚨

     

    Please ensure that you have uploaded the all of your evidence.

    The MUST include:

    • Your Name and Address.
    • The date of your Universal Credit payment.
    • EVERY page of any letter, payslip or other evidence that you are submitted.

    If there is ANY part missing from your evidence it will delay your Legal Aid assessment and appointment. 

  • Should be Empty:
