I understand that I will be notified in the case of a medical emergency involving my child(ren). In the event that I nor emergency contacts can be reached, I authorize the calling of a doctor and the providing of necessary medical services in the event my child(ren) is injured or becomes ill. 

I understand that Moving Along Transportation WILL NOT be responsible nor will they reimburse any medical expenses occured, but that such expenses will be my responsibilty as parent/gaurdian. 

Additionally, I agree to keep my child(ren) medical conditions up to date with Moving Along Transportation should any changes occur.

I hereby give permission for the transportation of my child to be provided by Moving Along Transportation Services.

** IN ORDER TO ENSURE ALL STUDENTS ARE PICKED UP AND DROPPED IN A TIMELY FASHION, THERE IS ONLY A 5 MINUTE GRACE PERIOD IN WHICH WILL WAIT FOR YOUR STUDENT(S). AFTER 3 LATE WARNINGS, WE WILL TERMINATE YOUR CONTRACT AND SERVICES.

All payments are due FRIDAY BEFORE THE UPCOMING WEEK. Payments can be made online via our website. Payments not made by SUNDAY will inquire a $15 late fee and could potentially intervene with your child(ren) being on our pick schedule. After 3 late payments, Moving Along Transportation Services serves the right to terminate contract. 

Moving Along Transportation Service is a private, professional transportation service that safely transports students to and from school, home, after school programs & activities, parents' jobs/worksites, a relative’s home and/or to a destination of your choice (within our pick up and drop off radius), for a weekly rate. Moving Along Transportation takes no shortcuts on safety. Thus, all of our drivers go through background checks, have safe driving records, and love working with children.

We are available for morning or afternoon pick-up and drop-off services. Morning (A.M.) bus service begins at 6:00 a.m. Afternoon (P.M.) service begins at 1:45 p.m (except for early release days where we will be available during the schools release time). It is our desire to pick up and drop your child off safely to, and from, all destinations. We strive to pick-up each student within 30-45 minutes of their school's dismissal time and have them to their destination within an hour of the time that they are picked up.

Moving Along Transportation Services is not responsible for any lost or damaged personal property.  

I understand the no refunds or transfers unless due to illness or other unforseen circumstance (proper supporting documentation will be requested)
