Please note that this is only a request form. You are not yet booked in as I require a non-refundable £100 deposit. The information concerning this is detailed below. Once paid you will be contacted by email to confirm a booking date and time
Please read before submitting this form:
• In order to confirm an appointment I require a £100 non-refundable deposit. This needs to be paid to confirm a booking date

• On the day your appointment you’ll come in at 10am well sit down together and design everything on the day. Unless you would like to see a design prior which will incur further costs. Can be discussed further if required

• However designing on the day is preferred. This way we can collaborate and work through the design process together. You have a direct input on the overall outcome of your design overseen by my professional guidance. It must be noted that I do not charge for any of this time

• Once we are both happy we can get stuck in and crack on with the work at hand

• You will only be charged for the time under the machine as I said I do not charge for any design time nor for the lunch break which is generally around 2/2:30pm

• My hourly rate is £125. I work from Tuesday to Saturday 10am till 5pm and tattoo for however long is requested

• Deposits are £100 and are non refundable. They are to confirm your booking and will be taken off the overall cost of your session

• If your unable to make your appointment please let me know in advance (72 hours before the date of your booking). Failing to do so will result in your deposit being lost

• Show up on time to your appointment and let me know if you are running late

• Make sure that you have not consumed drugs or alcohol 24 hours before your appointment as this may affect the tattoo process

• Please make sure you have eaten something substantial before hand. It is advised to bring some sugary snacks along with you to your appointment

• You must bring a form of photo identification. We accept passports and drivers licenses only. You must be over 18

• We accept bank transfer or cash for the final payment on the day of your appointment. Payments must be made before leaving the studio
