Royal Rebels Waiver

Royal Rebels All Stars Waiver and Release of Liability

PLEASE READ CAREFULLY BEFORE SIGNING, THIS AFFECTS YOUR LEGAL RIGHTS.
In consideration for being allowed to participate in the activities offered by Royal Rebels All Stars, I hereby irrevocably and unconditionally agree for myself, my personal representatives, my heirs, next-of-kin, insurers, successors, and assigns, as follows:

1. Assumption of Risk

I understand that participating in activities including but not limited to stunting, tumbling, trampoline, all-star teams, birthday parties, recreational classes, cheerleading, and other related activities (collectively referred to as the 'Activity') organized by Royal Rebels All Stars at 20268 NE 15 Court, Miami, Florida 33179 ('Premises') involves inherent risks. These risks may include, but are not limited to, injuries such as sprains, fractures, contusions, dislocations, paralysis, or even death. I acknowledge that these risks cannot be eliminated without jeopardizing the essential qualities of the Activity.

2. Release from Liability

I voluntarily and knowingly assume all risks associated with participation in the Activity. I hereby release, waive, and discharge Royal Rebels All Stars, its affiliates, employees, agents, and volunteers from any and all claims, liabilities, or damages arising from injury, loss, or death resulting from my participation in the Activity, including those caused by the negligence of the Company.

3. Indemnity

I agree to indemnify, defend, and hold harmless Royal Rebels All Stars and its affiliates from any claims, damages, or expenses arising from my participation or the participation of the Minor(s) in the Activity.

4. Covenant Not to Sue

I agree not to file or assist in filing any lawsuit or legal action against Royal Rebels All Stars for injuries, losses, or damages arising from my or my child's participation in the Activity.

5. Medical Consent

I authorize Royal Rebels All Stars to obtain emergency medical treatment for me or the Minor(s) if necessary. I acknowledge that I am responsible for any medical expenses incurred.

6. Equipment and Facilities

I will immediately report any unsafe conditions to Royal Rebels All Stars staff and will not participate until the issue is resolved.

7. Representation

I confirm that I am over 18 years of age, in good health, and physically able to participate in the Activity. I am not pregnant and do not have any medical conditions that could be aggravated by the Activity.

8. Effective Date and Duration

This waiver is effective from the date of signing and remains valid for the duration of my enrollment and participation in Royal Rebels All Stars activities.

9. Severability

If any portion of this waiver is found to be invalid or unenforceable, the remaining sections shall remain in full force and effect.

By signing below, I acknowledge that I have read, understood, and agree to the terms of this waiver. I voluntarily give up my right to sue Royal Rebels All Stars for any injuries or damages resulting from my participation.

I certify that I am the parent or natural guardian of the child listed below and that I have authority to sign this waiver on his or her behalf. I certify that I have adequate medical insurance to cover any injury or damage that my child may cause or suffer, or else I agree to personally bear the costs of such injury or damage and indemnify. I release the Company from all bills, losses, liabilities, claims, obligations, costs, damages or expenses that may result from my child’s participation in the Activity or use of Company's property. I certify that my child is in good health, in proper physical condition, has the skill level required to participate in the Activity, and that he or she does not have any medical or other conditions that would impair his or her ability to participate in the Activity. My child does not have any ailment that could be aggravated by his or her participation in the Activity.
