We do not spray women who are in the first trimester for safety reasons. If you are beyond the first trimester or nursing, written permission must be obtained from your doctor before being airbrushed tanned.

There are occasions where individuals may be allergic to one or more ingredients in the spray tan solution.Our machine has a latex rubber hose and individuals allergic to latex should not be spray tanned.There is a very small percentage of people whose skin does not react favourably to spray tanning. For this reason we do NOT advise being spray tanned for the first time when your appearance is critical (for ex., wedding, photographic session, modeling assignment etc.) We advise that you come in at least one time prior to your special event.Some fabrics may be stained by the spray tanning solution, please use care.This product does not contain sunscreen and does not protect against sunburn.I hereby consent to receiving spray tanning services. I hereby waive any and all claims or rights on my behalf as a result of any service rendered and release and forever discharge and its employees from any and all claims and responsibility or cause of action arising from the rendering of tanning services and or treatments.

To my knowledge, I have no medical conditions or allergy which would preclude me from having this procedure done. I agree to waive any and all medical complications that may arise. I have been advised to discontinue use and consult a physician if any reaction occurs.

I, the undersigned agree to and understand the following:*

I have understood all of the instructions & procedure details given to me prior to the session.I understand that I am responsible for all jewellery, clothing, accessories I wear before & after my tanning session in case of staining or damage.While allergic reaction is rare, I assume responsibility for any kind of allergic reaction I may have and will seek immediate medical attention and absolve the airbrush technician of any liability for the application of the DHA sunless tanning product.While the DHA bronzing residue will appear darker after four to six hours the client understands that the cosmetic portion of the tan will " wash off " with their after tan shower leaving a natural looking tan,that can vary in the length of time it will last from one day to 3 – 5 days..I understand that DHA has been FDA approved for sunless tanning for over 30 years.I understand that I am doing this at my own risk.If under the age of 18 years, you must have a parent or guardian's consent before being airbrush tanned.

Parental Consent

I Parent's Name being the legal guardian of Client's Name a minor do hereby acknowledge, agree and consent to her or him receiving spray tanning services or treatments without my presence. I hereby waive any and all claims or rights on behalf of my child as a result of any service rendered and release and forever discharge the salon and its employees from any and all claims, responsibilities or cause of action arising from the rendering of spray tanning services and or treatments to, Name , my child. I have read and completely understand this consent form.
