STATEMENT OF VOLUNTRY CONSENT, GENERAL RELEASE
AND
WAIVER OF LIABILITY

In consideration of my voluntary participation in Main Street Zephyrhills Halloween Howl 2023, and for good and valuable consideration received by me, I assume all responsibility for any risks or injuries inherent in participating in Halloween Howl 2023. I further hereby hold harmless and release and forever discharge The City of Zephyrhills, Main Street Zephyrhills and all of their officers, agents, volunteers, employees, and their successors, for any and all claims and demands whatsoever, which the undersigned, and their heirs, representatives, executors, administrators, and personal representatives therefore, or any person acting in behalf of their respective agents, have or may have against any or all of the aforementioned person or their successors, by reason of accident, illness, injury, property loss or damage or any other consequences arising or resulting directly or indirectly from any participation in the Main Street Halloween Howl program or activities.
I hereby declare and represent that in making, executing and tendering this State of Voluntary Consent, General Release and Waiver of Liability, I fully understand and acknowledge by my signature, that I am relying wholly upon my own judgement, belief and knowledge of the circumstances involved in my participation in the above described activity, and I have read the Statement, understood its contents, and execute it of my own free will and choice.

I also understand and agree that I will complete the Volunteer Release and Waiver of Liability Form prior to any work or volunteering services being performed.
