Release of Liability
Although the safety of all sport activities is the primary concern, indoor sport activities at Tucson Basketball Factory's facilities may cause injuries and/or death. I expressly assume the risk of injury, death, and/or illness arising from any cause, and agree to waive the right to pursue any claim against the Tucson Basketball Factory and the persons in charge.
Participant Waiver & Release of Liability

I, the parent or legal guardian of the participant listed below, acknowledge that participation in volleyball activities involves inherent risks, including but not limited to physical injury, illness, or accidents that may occur during practices, games, training sessions, or related activities.

I voluntarily allow my child to participate in all Tucson Volleyball Factory programs, including but not limited to development leagues, clinics, practices, games, and events. I understand and accept all risks associated with participation.

I hereby release, waive, and discharge Tucson Volleyball Factory, Sports Factory, its directors, coaches, staff, volunteers, facility partners, and affiliates from any and all claims, demands, actions, or causes of action arising out of or related to my child’s participation, including injury, illness, or property damage, whether caused by negligence or otherwise.

I certify that my child is physically able to participate in volleyball activities and has no medical conditions that would limit safe participation, except as disclosed in writing prior to participation. I understand that it is my responsibility to notify program staff of any medical concerns or special needs.

In the event of an emergency, I authorize Tucson Volleyball Factory staff to seek medical treatment for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.

I also grant permission for Tucson Volleyball Factory to use photographs or videos of my child taken during program activities for promotional, educational, or marketing purposes, unless I submit a written request opting out.

By registering my child and signing below, I acknowledge that I have read, understand, and agree to the terms of this waiver and release.
