1. Payment plans: Please explain in detail the reason why you are in need of a one semester, non-renewable monthly payment plan. Provide documentation of your situation below.

If offered a monthly payment plan, how do you intend to pay for your monthly tuition? (please describe source of income, funding, or additional resources you will use).

2. Emergencies: For a refund due to a medical or other emergency, please describe your situation below. Note: Tuition is only refundable before the July residency begins. Commitment deposits are unfortunately non-refundable.

3. Other situations or requests: Please explain here.

Documentation: Related documentation is required for all requests (i.e. Doctor's note stating you are unable to attend for health reasons, documentation of an emergency situation, etc.)
