To be completed by the student for each class subject hour of tutoring time.
Fill out this form to ATTEND or to CANCEL a tutoring session If you know that you are not going to be attending your already scheduled online learning session, and are needing to cancel it, then please fill out this form at least two hours in advance prior to the online learning session's starting date and time.

I understand that emergencies may come up, for this reason I ask for your cooperation with communicating it with me so that I may accommodate all students, accordingly. I appreciate you contacting me and letting me know. Thank you!
