本人明白越野跑運動具有一定風險，可以導致嚴重身體受傷或死亡；本人聲明參加者的健康及體能良好，適宜參加上述活動；本人明白及同意會遵守所有有關之活動規例和教練之臨時安排及倘於活動期間發生意外，包括死亡、受傷或財物損失，將自行承擔一切責任；主辦機構及教練無須對此作出賠償或負上任何法律責任；本人亦授權予舉辦者使用任何有關此訓練其間拍攝的之相片，錄像及紀錄作任何合法用途。
