STEAM ACHIEVERS AND SNAC EDUCATIONAL SERVICES PROGRAMS, PURPOSE HACKATHONS, HACKAUTISM, S.T.E.A.M. FIELD TRIPS, WORKSHOPS AND EVENTS
WAIVER OF LIABILITY AND CONSENT FORM
I understand that in order to participate in any STEAM Achievers and S.N.A.C. Educational Services event, activity, field trip, STEAM Achievers HackAutism Powered by Microsoft, at Microsoft Corporation, and any other sponsored, co-sponsored, in-school, out-of-school, corporate partnership event, online webinar, online class, event within or outside the members of my school community, local community or business community, I must sign this Waiver of Liability Agreement (Waiver) and Consent form. For purposes of this Waiver, the term “STEAM Achievers” shall include STEAM Achievers, Uplift North Hills Prep, Uplift Education, S.N.A.C Educational Services, Microsoft, University of Texas at Dallas, University of Texas Southwestern, Alliance of Technology and Women, National Autism Association of North Texas, UNT Kristin Farmer Autism Center, Wolters Kluwer, and other participating organization in person, in-kind, on-site, off-site, before or after event, and, its parents, subsidiaries, affiliates, related companies, and all of its directors, officers, agents, representatives, volunteers, employees, licensees, partners, sponsors, permittees, assigns and successors in interest. By participating in the Hackathon, I understand that I assume all responsibility and risk associated with all conditions, hazards and potential dangers associated. I release, discharge, and waive any claims, actions, or suits of any character, name and description that I may have against STEAM Achievers, Uplift North Hills Prep, Microsoft and any above-named and unnamed partner as a result of any injuries, damages or death received or sustained. I also understand that the University of Texas at Dallas (Computer Science) will not take custodial responsibility of minors while participating in any "STEAM Achievers Purpose Hackathons" and all related events and activities. Custodial responsibility will remain with teachers (K-12), parents, legal guardians, and chaperones.
MEDIA CONSENT AND RELEASE
For valuable consideration, the receipt and adequacy of which I acknowledge, I irrevocably give to "STEAM Achievers" as defined above, Microsoft, the perpetual, worldwide, exclusive license, right to copyright and/or trademark in the name of STEAM Achievers and Microsoft, and to use in any form or fashion, including, but not limited to, advertising, publicity and all other purposes deemed appropriate by STEAM Achievers and Microsoft, my child’s name, signature, picture, image, likeness, voice, poses, plays, appearances, movements, or any other indicia of my child’s identity or activity of any nature created, depicted, captured or recorded by or at the direction of STEAM Achievers and Microsoft (collectively, “Materials”) in or in connection with any and all media of any kind and nature now known or developed in the future (collectively “Media”), for any purpose, in any manner, without further notification, authorization or compensation to me, my child or anyone acting on our behalf. I understand that STEAM Achievers and Microsoft may use all Materials at their sole discretion and that my child and I do not have any right to inspect or approve the use of the Materials in any Media. STEAM Achievers and Microsoft, may transfer all of the rights granted by this Media Consent and Release. I, on behalf of myself and my child, further waive, assign and release to STEAM Achievers and Microsoft all rights associated with the materials and release STEAM Achievers and Uplift North Hills Prep, from any liability associated with the materials or use of the materials in any media.
I grant STEAM Achievers, S.N.A.C. Eductional Services, Microsoft and Uplift North Hills Prep, permission to participate in media events including but not limited to WFAA ABC News, Dallas Business Journal, NBC, Irving Community Television Network (ICTN), Star Local Media, public media, social media including but not limited to Facebook or YouTube, for promotional or educational purposes.
CODE OF ETHICS AND RIGHTS OF OTHERS
Absolute respect for the rights of others is expected of all members of the school. Rudeness, profanity, socially-offensive language, or physical or verbal malicious behavior to faculty, staff, or peers is not acceptable. Dishonesty, lying, insolence, profanity, verbal abuse, harassing (including sexual harassment and any sort of harassment by phone, mail, Internet, or e-mail), displays of affection that embarrass others, hazing, and physical abuse or acts endangering the safety of others are regarded as serious breaches of conduct and will result in serious disciplinary action that may involve intervention from outside agencies.
Academic disruption, which includes inappropriate behavior inside or outside any class in session, will be addressed according to school rules with the student’s individual behavioral goals in mind. In addition to respecting the property belonging to the school or to others inside and outside of the campus including out-of-school event locations, students must also respect the property of homeowners and businesses in the community. Stealing or vandalism in any form, including the unauthorized “borrowing” of another’s belongings, which includes entry into another’s locker, purse, book bag, or other personal item; removing food illegally from the student store; removing computer, electronic devices, power strips, cords, speakers, projectors and any other equipment without permission; writing or carving on desks, walls, bulletin boards, or other property is unacceptable.
MY USE OF SCHOOL COMPUTING RESOURCES
· I will help to create a positive atmosphere by allowing those engaged in academic work priority use of the computers. · I will respect the work and privacy of others throughout the Uplift Education network. · I will use my applications, e-mail accounts, and Uplift Education network space appropriately for school-related activities. · I will not save or install files and/or software on School equipment without the authorization of a teacher or the network administrators. · I will not use Uplift technology resources for commercial activity, for seeking monetary gain, or for political purposes. System Security · I will log on to the network only as myself. Scholar Handbook 2016-2017 73 · I am responsible for my individual account and will take all reasonable precautions to prevent others from being able to use my account. · I will immediately notify a faculty or staff member if I have identified a possible security problem. · I will not intentionally introduce a virus or other harmful code anywhere on the Uplift Education network, and I will make an effort to keep my home computer free from viruses and other destructive materials. If my files are accidentally infected, I will seek help from a member of the technology staff. · I know that any electronic devices brought on this campus are subject to search without notice or warning. I will refrain from using any device or software that masks my use of the school resources. This includes but is not limited to anonymizers and any application or hardware device that circumvents network security, logging, or tracking procedures.
EXPECTATIONS FOR SCHOLAR CONDUCT
Each scholar is expected to: · Behave in a responsible and respectful manner; · Demonstrate courtesy and respect for others, especially other scholars and School personnel; · Attend classes and required School activities and events regularly and on time; · Prepare for each class and complete assignments on time; · Respect the rights and privileges of other scholars and school personnel in person and online; · Respect and care for School property and facilities; · Turn off cell phones or other electronic media during instructional time, and otherwise follow policies related to use of technology; · Cooperate with School personnel in maintaining safety, order and discipline; · Dress appropriately in accordance with the School’s dress code; · Review and comply with the Scholar Handbook and other School and campus rules; · Obey classroom rules and classroom expectations for behavior; · Refrain from verbal or written acts of bullying whether in person or online; · Avoid violations of this Code. Scholars are expected to comply with the school’s technology and electronic media use policies and procedures. Scholars are also expected to demonstrate the same behavior online or while using electronic media as is expected in the classroom, on school property, or at school-related events. Sending, posting, or possessing electronic messages that are disrespectful, disruptive, abusive, obscene, illegal, threatening, harassing or damaging to another person disrupts the learning environment and will not be tolerated. Parent cooperation is essential to achieving the School’s mission and ensuring that scholars learn in a safe, secure and positive environment. Parental involvement and cooperation is vital in the discipline process. Parents should understand and be familiar with the Code, ensure that his or her child understands and seeks to comply with this Code, and assist the school in counseling scholars regarding appropriate behavior.
EMERGENCY DISASTER PLAN AND RECOMMENDATIONS
Parents are instructed to follow the important procedures listed below in the event of an emergency disaster.
1. Tune in to local emergency radio station when you arrive in a safe area and whenever safely possible, with the use of the utmost precautionary measures.
2. Stay off the telephone. Trying to communicate with the school via telephone after a disaster will prove difficult if not impossible.
3. After an emergency, students will be escorted to an emergency assembly area, which is located on the field at the center of the campus.
4. When arriving at school, parent or other adult designated by parent(s) should come to the lobby in the Administrative Building.
5. The school administrator or staff communication team will be available in the lobby to assist parents.
6. No child will be dismissed from the emergency assembly area without the authorization of the parent and/or principal.
7. Parents are asked to wait in the lobby. Parents will remain there until their child is escorted to them.
8. The school will remain open until every child has been released to his/her parent(s) or to authorized person(s).
9. Parents and/or parent designees are advised to remain calm and to follow the emergency disaster procedures which have been established at the school site or by emergency officials.
By signing this, I hereby confirm I have read and agree to all of the above rules, code of ethics, use of computing resources, expectations of scholar conduct, authorizations, permissions, releases as indicated and the authorization as indicated.
