Membership Cancellation (Annual)

This is a formal request to cancel your Smart Fit Method gym membership. In cancelling your membership, you will no longer have access to Smart Fit Method provided services including use of the facility, use of the client app and access to our nutritionist.

Cancellation request must be submitted 2 weeks prior to next billing date
The Member agrees to pay a contract breakage fee of $600.00 as outlined in the signed Annual Membership agreement.
Cancellation of subscription will start at the end of the billing date it was requested
For monthly payments, there will be no refund after cancellation
Cancellation will only be accepted via this form and not via phone or email.
