I agree and I acknowledge that I have truthfully presented valid proof to True Glory Tattoo Co to be over 18 (eighteen) years of age.
I agree and I acknowledge that I am free of communicable diseases.
I agree and I acknowledge that I am not pregnant or breastfeeding.
I agree and I acknowledge that it is not reasonably possible for True Glory Tattoo Co to determine wether I might have an allergic reaction to the dies, pigments or process used in my tattoo. I also agree to accept the risk that such a reaction is possible.
I agree and I acknowledge that I need to take care of the tattoo by following the instructions given to me by the Tattoo Shop.
I agree and I acknowledge that infection is always possible as a result of obtaining a tattoo, particularly in the event that I do not take proper care of my tattoo.
I agree and I acknowledge that my tattoo may cause scarring.
I agree and I acknowledge that I am obtaining the requested tattoo at my own free will and consent to the application of the tattoo and to any actions or conducts by “The Artist” to reasonably perform the tattoo.
I agree and acknowledge that all tattoos on fingers, hands, feet, and neck are not guaranteed.
I confirm that the information I provided in this document is accurate and true.
TATTOO AFTERCARE INSTRUCTIONS

NO BEACH - NO SUN - NO POOL FOR 2-3 WEEKS AFTER GETTING YOUR TATTOO
- Do not scratch your tattoo or pick at scabs, as this may result in loss of ink.
- Keep your tattoo clean with antibacterial soap & warm water.
- Pat dry with paper towels.
- Keep your tattoo moisturized with alcohol free & fragrance free lotion (Aquaphor). Apply it in small amounts 2-3 times a day as needed.
I agree and acknowledge receipt of written instructions advising me of proper care of my tattoo and I recognize the absolute necessity to follow those instructions.
