It is a pleasure to be able to guide you on your yoga journey. To ensure the safety of you and others, please complete this short release of liability form.

Liability Waiver and Agreements:

Age Verification

For Adult Participants: I affirm that I am at least 18 years of age.
For Youth Participants: I affirm that I am the parent or legal guardian of the youth participant named above.

Sick/Covid-19 Policy

I affirm that neither I (for adult participants) nor my child (for youth participants) will attend in-person if we have tested positive for COVID-19 within the past 14 days or have been exposed to anyone diagnosed with COVID-19 within the past 30 days.
I understand that it is my sole responsibility to actively use best practices when it comes to COVID-19 or any other contagious illness.
I agree to provide at least 24 hours notice if cancellation is needed due to illness. I understand that I will be able to reschedule free of charge if I cancel at least 5 hours prior to the scheduled appointment.

Understanding of Yoga Practice

I fully understand that yoga is considered to be a physical and mental activity.
I understand that it is important and my responsibility to consult a healthcare provider before engaging in any type of activity, including yoga, to ensure it is safe for me or my child to practice such activity.
I understand that the yoga studio strives to create a comfortable, respectful, and inclusive environment for all practitioners and agree to the dress policy where all participants, regardless of gender, are required to wear both tops and bottoms that allow for free movement yet provide sufficient coverage during yoga classes and activities.

Holistic Practices Acknowledgment

I understand that holistic practices such as yoga, meditation, or life coaching are not meant to evaluate, diagnose, or treat medical conditions and such services do not act as a substitute for medical treatment.

Assumption of Responsibility

I agree to assume full responsibility for my participation (or my child's participation) with this service and any future services offered by or through YogaSoul.Online, Christina Baggott, Yoga Soul Wellness Center, and all of its networked affiliates connected to in-person or streamed classes, training, or workshops.

Release of Liability

I hereby agree to irrevocably release and waive any claims that I have now or may have hereafter and hold harmless YogaSoul.Online, Christina Baggott, Yoga Soul Wellness Center, and all of its networked affiliates connected to in-person or streamed classes, training, or workshops.
