WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19

ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT

In consideration of being allowed to participate on behalf of Generals Athletic Club program and related events and activities, the undersigned acknowledges, appreciates, and agrees that: 1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19 While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and, 2.I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, 3.I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, 4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (Generals Athletic Club, INC) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

Acknowledgement of risk of injury clause: "As a participant in the program, I recognize that there are risks of physical injury and I agree to assume the full risk of any injuries including death, damages, or loss which I may sustain as a result of participating in any and all activities connected with or associated with this or such program"

Wavier of claim for injury clause: "I agree to waive and relinquish all claims I may have as a result of participating in the program against the Generals Athletic Club, Inc. (GAC, Inc and it's officers, agents, servants, employees, volunteers, any and all GAC, Inc. affiliates including Shiloh High School, Briscoe Park andits staff and properties

Release from liability clause: "I do hereby fully release and discharge the Generals Athletic Club, Inc. (GAC, Inc and its officers, agents, servants, employees, volunteers any and all GAC, Inc. affiliates including Snellville Middle School, Shiloh High School, Briscoe Park, its staff and properties from all and any claims including death, damages, or loss which I may have, or which may occur to me on account of me or my child's participation in the program"

Photography/Media Release for Minor Child or Children

I hereby authorize Generals Athletic Club, hereafter referred to as "G.A.C Sports", to publish photographs taken of myself and/or the minor child or children listed below, and our name likenesses, for use in the Generals Athletic Club print, online and video-based marketing materials as well as other Company publications.

I hereby release and hold harmless Generals Athletic Club from any reasonable expectation of privacy or confidentiality for myself and for the minor child and children listed below associated with any images in possession of Generals Athletic Club. Further, I attest that I am the parent or legal guardian of the child or children listed below and that I have full authority to consent and authorize Generals Athletic Club to

use their likenesses and names.

I further acknowledge that participation is voluntary and that neither I, the minor child, or minor children will receive financial compensation of any type associated with the taking or publication of these photographs/videos or participation in company marketing materials or other company publications. I acknowledge and agree that publications of said photos/videos confers no rights of ownership or royalties whatsoever.

GAC Parent Code of Conduct
The Generals Athletic Club will strive to be the best, in providing a positive Sportsmanship Environment for our players. Our program is designed to create a positive community atmosphere and enjoyable culture.

1) Encourage the participating athlete in competing to his/her best ability 2) Keep all comments positive. 3) Only players, coaches and staff are allowed on practice and game field, no exceptions. 4) Leave the coaching to the coaches. 5) Taunting the officials, coaches, staff, and opponents will not be tolerated at all. This is youth sports, everyone makes mistakes, and officials do too. 6) Explain mandatory conduct policy to all your family members and invited guest attending practices and games in support of your child athlete. 7) No open alcoholic beverages on fields or parking areas. 8) Help make this a positive and fun experience for all, ESPECIALLY THE KIDS. 9) Report any violations or inappropriate activity to head coach or GAC staff, get involved. 10) Each player is allowed a minimum play time. Playing Time is not to be discussed with the coach during any GAC game or event. Play time concerns are to be discussed with Head Coach no later than 48 hrs. prior to game day event and/or no sooner than 24 hrs. (cool down period) AFTER the game/event. 11) Clean up behind yourself in your designated spectating area. Check for personal belongings. GAC is not responsible for your loss of items. 12 Enjoy watching your child have fun, socialize, learn, and develop. 13 Remember that you are in a kid friendly environment.

1) Individuals will be given one fair warning for inappropriate behavior/activity. 2) Individuals that continue; will be asked to leave or escorted off the fields/stadium Players can/ will be removed from the game until the spectator has left. 3) Other sanctions as deemed necessary by authorities may be enforced, including permanent ban. 4) All decisions are final; there will be no hearing or appeals. 5 Zero tolerance for verbal and/or physical violence. IMMEDIATE REMOVAL WILL TAKE

PLACE IN THE EVENT OF AN OCCURRENCE.

SCHOOL GROUNDS Georgia Law has zero tolerance for Guns or Drugs on school grounds! If you violate these laws, you will be subject to prosecution of a Class A Felony! THIS MUST BE SIGNED AS A CONDITION OF YOUR CHILDS PARTICIPATION
