Please take the time to thoroughly fill out this application form. Failure to do so will be an automatic rejection of your application.

This application should ONLY be completed in the event that a puppy is available from our website. THIS IS NOT A WAITLIST APPLICATION. There is a separate application form for our waitlist which can be found on our website.

Puppy Preference
Please be certain that the puppy that you are applying for is available. If the "Reserved" or "Sold" sign is placed on a listing it means that the puppy has already been claimed and cannot be applied for.

Moreover, please indicate the "Name" of the puppy you are applying for from our website in the next question following this note. Neglecting to place a name here will automatically reject your application.
