NDP Family Training and Navigation Service
    Professionals' Referral Form
  • We welcome referrals to this service from professionals who are working with a child/young person in Brighton & Hove or East Sussex on the neurodevelopmental pathway (NDP).

The service aims to provide local parent carers with information and training, as well as signposting them to support networks that can help sustain them through the challenges these conditions (and commonly related diagnoses) bring to a family.

If the family are seeking information, advice and support on specific SEND education, health, or social care matters or about claiming DLA or PIP, please direct them to Amaze SENDIASS service.

Before you submit the form, please read the information below and the following link for further information on the NDP Family Support page.

We aim to make initial contact with the parent carer within 10 working days of your submission, and our working hours are Monday to Thursday 9.30am-3pm.

  • Please note we can only support families living or with a GP in Brighton & Hove or East Sussex.

  • East Sussex Criteria for referral

    • The child/young person and their families must be registered with an East Sussex Brighton & Hove GP and/or resident in East Sussex
    • They must be under 25 years old at the time of referral
    • They must be at some point on the NDP diagnostic pathway for assessment for Autism Spectrum Condition (ASC), ADHD (Attention Deficit Hyperactivity Disorder), Foetal Alcohol Syndrome, Learning Disabilities/Developmental Delay, Social Communication concerns, Sensory Processing Disorder, or Tics /Tourette Syndrome.  See further information below.

Points on the NDP when you can refer

    • The earliest time you can make a referral is at the point a parent carer is looking into getting a referral for assessment by the NHS
    • We can accept referrals at the point of referral, whilst the child/young person is waiting for assessment, in between assessments, when a diagnosis has been made, or no diagnosis has been made but traits of one of the neurodevelopmental conditions have been confirmed.
    • You can also refer post-diagnosis, at times of key challenges or change such as preparing for adulthood, transitioning between stages of education, puberty, when behaviours are becoming tricky, or a child/young person has emerging mental health issues.

    We can accept referrals from

    • East Sussex Hospital Trust (ESHT)
    • CAMHS (Child Adolescent Mental Health Service)
    • GP practice
    • A health visitor or school nurse
    • A children’s centre
    • A child/young person’s school, college, or nursery
    • ISEND
