THIS FORM IS FOR INSTRUCTORS OF OTHER INSTITUTIONS ONLY.
STAFF ONLY:

This form is for LBCC Testing Services to proctor a NON-LBCC exam for outside institutions.

LBCC Testing Services will need to receive proctoring directions before the student is allowed to schedule their tests.

Students will be required to show a photo ID prior to testing.

Once you have submitted this form with your student's name, test and proctoring directions, please give your student this link to schedule their testing appointment. They must schedule their appointment at least 48 hours before their test.
