PROGRAM/EVENT ATTENDANCE & ACTIVITY SAFETY.
Thank you for your support of Believe In A Dream Inc., our mission, and our programs. As we continue to navigate the COVID challenges, we have returned to our in-person, event-based programming – offering our young participants the opportunity to continue to build their musical and artistic skills, explore their individual strengths and personal brands, develop their leadership style, and grow their entrepreneurial knowledge – with safety first in mind.
Our immersive programming includes concerts and performances, leadership summits and conferences, annual fundraisers, student-led events and gatherings, and in-person meetups and rehearsals – all of which bring together dozens of young people throughout our community. Many of our events and activities have a strict schedule (arrival time, departure time, lunch & break times, etc.), while others are purposely less structured — so as to create an open-house, come-and-go environment that allows young participants to attend for as long or as little as they please. Event information and specifics are communicated to young participants and parents/legal guardians via email and Remind text-alerts, prior to our events. (We recommend all parties – students and parents/legal guardians – register under the correct Remind code and opt-in for ext alerts.)
Prior to most of our events and activities, we ask young participants, and occasionally their parents/legal guardians, to register by completing a special event online form. This form is in addition to the longer, more-detailed program registration form. Often, we also create a separate Remind text-alert group for special one-time events, in order to communicate updates or unanticipated, last-minute changes.
During our in-person events and activities, we ask that young participants check-in when they arrive to the event location, and check-out when leave the event location. Most times, we have a registration station set up with a designated staff member or volunteer working to sign-in and sign-out students. In addition to signing-in and signing-out, we ask, but cannot always enforce, young participants to remain at the event location under the supervision of our staff and volunteers. With certain public events, we even hire an off-duty police officer to provide additional safety and security. Although we take safety precautions at our events, Believe In A Dream Inc. is not responsible for the safety and well-being of your child before arrival to the event location, after departing the event location, or if your child chooses to leave the event with or without our or your knowledge and permission. If your child plans to attend one of our events or activities, it is your responsibility to make sure your child does not leave the event location, unless your child is departing the event. We want to ensure the safety of all young participants while attending any Believe In A Dream Inc. events and activities.
SPECIAL EVENT: Downtown Scavenger Hunt
During the upcoming student-led Scavenger Hunt event, scheduled for May 13, 2023, teams of young participants will solve riddles and walk around in groups within the Downtown Fort Wayne vicinity to collect specific items from various locations, while learning about the local community. We plan to assign at least 1 adult volunteer to each group of young participants, to serve as a supervisor. However, if you agree to allow your child to participate in this event, please note that although we will take steps to reduce risks to participants, you assume all risk for your child’s safety throughout our event. We welcome you to join your child during this Downtown Scavenger Hunt, if you would feel more comfortable.
Your signature acknowledges that you understand and accept the statements above, in addition to the statements below.
ACKNOWLEDGEMENT AND ASSUMPTION OF RISK.
I acknowledge that my child derives personal satisfaction and a benefit by virtue of my child’s participation and/or voluntarism with Believe In A Dream Inc., and my child willingly engages in Believe In A Dream Inc. events or activities.
I also acknowledge that Believe In A Dream Inc. is not responsible for the safety and well-being of my child, before my child arrives to a Believe In A Dream Inc. event or activity, after my child departs a Believe In A Dream Inc. event or activity, or if my child chooses to leave during a Believe In A Dream Inc. event with or without my knowledge and permission or the knowledge and permission of Believe In A Dream Inc. staff and volunteers.
As parent/legal guardian, I assume all risks of my child attending any Believe In A Dream Inc. event or activity.
LIABILITY RELEASE WAIVER.
I HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE ANY AND ALL LIABILITY, CLAIMS, AND DEMANDS OF WHATEVER KIND OR NATURE AGAINST BELIEVE IN A DREAM INC. AND ANY AFFILIATED PARTNERS AND SPONSORS, INCLUDING IN EACH CASE, WITHOUT LIMITATION, THEIR DIRECTORS, OFFICERS, EMPLOYEES, VOLUNTEERS, AND AGENTS (THE “RELEASED PARTIES”), EITHER IN LAW OR IN EQUITY, TO THE FULLEST EXTENT PERMISSIBLE BY LAW, INCLUDING BUT NOT LIMITED TO DAMAGES OR LOSSES CAUSED BY THE NEGLIGENCE, FAULT OR CONDUCT OF ANY KIND ON THE PART OF THE RELEASED PARTIES, INCLUDING BUT NOT LIMITED TO DEATH, BODILY INJURY, ILLNESS, ECONOMIC LOSS OR OUT OF POCKET EXPENSES, OR LOSS OR DAMAGE TO PROPERTY, WHICH I, MY CHILD, MY HEIRS, ASSIGNEES, NEXT OF KIN AND/OR LEGALLY APPOINTED OR DESIGNATED REPRESENTATIVES, MAY HAVE OR WHICH MAY HEREINAFTER ACCRUE ON MY CHILD’S BEHALF, WHICH ARISE OR MAY HEREAFTER ARISE FROM MY CHILD’S PARTICIPATION AT A BELIEVE IN A DREAM INC. EVENT OR ACTIVITY.
MEDIA RELEASE.
I hereby give Believe In A Dream Inc. permission to use, and/or copyright, personally identifiable information about my child including photographs, video, name, school of attendance, academic or personal achievements, testimonials, and/or art works for publication, advertising or other lawful purposes including, but not limited to, publication on Believe In A Dream Inc.’s website, social media pages, or printed materials. I understand that this information might be published on the Internet and therefore published worldwide.
