• Competitive Cheer Registration (CYO)2022-23

  • Season Information

  • Competitive Cheer program begins week of September 19 and runs through March 2023

    • Season consists of 2 cheer competitions, CYO home games (basketball season), Prep Bowl and other special events - TBD
    • Practices will be held on Mondays and Thursdays:  4-5:30pm

    ***Attendance at practices is necessary and expected.

    • Cost:  $150 includes Uniform Rental and Bodysuit which athlete will keep and can reuse next season
    • Additional outside costs - spanx, white cheer athletic shoes, hair bow. (est. $60 total).  If you have these items from last season, you can reuse unless you need size changes. 
    •  Coach Jen will talk to you about these items specifically.  Please do not purchase until you hear from her.

  • Cheer Uniform

    Cheer Uniform Shell and Skirt will be turned in at the end of the season. A $50 replacement fee will be charged to those that do not turn it in. Uniform bodysuit will be the property of the athlete and can be used in future season.
  • Athletic Code of Conduct:  I will conduct myself in a Christian manner, exhibiting respect, responsibility, fair play, good sportsmanship, and be a team player while representing the St. Patrick Athletics Program.  I will observe team rules while participating in St. Patrick Athletics sanctioned athletic events, contests, practices and games.  I understand that I am responsible for my uniforms and any other equipment belonging to St. Patrick Athletics Program and that I will clean and return my uniform at the end of the season.

  • Parent Code of Conduct:  I will provide a good example of Christian behavior by supporting my child in his/her compliance with the Athletic Code of Conduct and will lead by example in teaching sportsmanlike behavior.  I will support St. Patrick Athletics Program by meeting athletic volunteer obligations.  

  • Sports Physical

  • Athletes are required to turn in  a Sports Physical dated after April 15, 2022 in order to participate.  A hard copy can be delivered to office or it can be emailed to athletics2@stpatschool.org

     

    SPORTS PHYSICAL FORM

  • Medical Alerts and Waiver of Liability and Authorization for Emergency Medical Care

    I/We hereby authorize the Principal of St. Patrick School, or his/her designee, Athletic Director, coach or sponsor in attendance at any St. Patrick Athletics contest or event to select, secure and consent to necessary medical attention for my child resulting from injury, illness or accident requiring medical care while I/We are not in attendance. I/We hereby release St. Patrick Parish and St. Patrick School and such person from any and all liability on account of such selection or authorization and for any and all damages which may occur on account thereof.
    Parent(s) or legal guardian of above athlete must consent to Waiver of Liability and Authorization for Emergency Medical Care in order for his/her child to participate in St. Patrick Athletics Program.
  • Assumption of Risk

  • In consideration of my child being allowed to participate in this sport, I hereby agree on behalf of myself and my child, to release St Patrick School and/or Parish, the Roman Catholic (Arch)diocese of Detroit and Lansing, and any and all affiliated organizations, their employees, agents and representatives (Releasees) from any and all claims, including negligence, which may be asserted by me or  my child, or on behalf of my child, arising from or relating to my child’s participation in the sport.

    In the event this release on behalf of myself and/or my child is held to be invalid or unenforceable, I hereby agree to indemnify and hold harmless Releasees from any and all claims, including negligence, which may be asserted by me or my child, or on behalf of my child, arising from or relating to my child’s participation in the sport. This release of indemnification does not apply to claims for intentional misconduct or gross negligence; nor does this release or indemnification apply to the extent of commercial insurance coverage for any claim, but this Release or Indemnification shall apply to the extent of any self-insurance or deductible applicable to any claim.

  • CYO Sports Photo Approval

    I hereby grant the Catholic Youth Organization of the Archdiocese of Detroit and St Patrick School/Parish permission to use photos or videos of myself and/or child taken during a CYO Athletic/Agency event.  In addition, I realize photos taken may be published on our Facebook pages, newsletters, websites, etc.
