  • Allergen Testing| Please read the following carefully:

    While allergies to treatments are unusual, it is always possible to experience an allergic reaction to certain products, pigments, and adhesives. I recommend a patch test 24-72 hours in advance for all new clients to test for allergic reactions.

    (Scheduling a patch test may result in the rescheduling of the desired appointment. It is also strongly recommended to get a patch test if lashes are for a special event such as prom or a wedding.)

  • Please read carefully the following instructions:

    Extensions are individual synthetic material that is attached with adhesive to your own eyelashes, not to your skin.

    These lashes can stay up to 4-6 weeks with proper care and maintenance that depends on the individual, lifestyle, and cycle of eyelash growth. For best results lash fills are required every 2-3 weeks.

    Please note:

    • Fills booked at or over 4 weeks will be charged as a full set.
    • Fills booked with less than 30% lash retention will be charged as a full set.
    • Corrections are offered free of charge only within the first 5 days of application. Please contact your lash artist as soon as any issues occur.
    • Attending an appointment with heavy eye makeup or mascara on the lashes will negatively affect the lashing process and the end results. It is your responsibility to attend appointments  with clean lashes. There will be a fee of $10 added if a lash bath is needed to remove make-up build up.

    After implementation, please DO NOT:

    • Try to remove eyelash extensions on your own
    • Use a regular eyelash curler
    • Rub your eyes or lashes
    • Pull your lash extensions
    • Attend a sauna or hot yoga
    • Use mascara
    • Cut the extensions
    • Apply false lashes ontop of extensions

    By signing this appointment and consent form, I, hereby, acknowledge, consent, and agree to all of the following:

    • I have read and understood the instructions above.
    • The eyelash extension procedure requires synthetic eyelashes to be attached with an adhesive to my own natural eyelashes.
    • It is my responsibility to keep my eyes closed and be still during the entire procedure until the technician leads me to do the opposite.
    • There are some risks of this procedure including but not limited to eye redness and irritation. The fumes from the adhesive may cause my eyes to tear up if I do not keep my eyes closed.
    • I understand that an allergic reaction to the adhesive or other products used during the service can happen at any time.
    • I disclosed any allergies that I may have.
    • I hereby release any and all employees or persons respresenting this establishment from all demands, claims, or damages, action or cause of actions arising out of the performance of this service.
    • It is my responsibility to inform my lash artist of any relative medical conditions (i.e. stye, pink eye, blepharitis etc), unusual retention, or irritations that may occur beofere during or after application.
    • I understand that all sales are final and no refunds are offered for services rendered, any fees, or any products sold.
    • I need to follow the after-care instructions and for any unexpected circumstances due to not following these instructions are at my own risk.

     

    I confirm to the above and agree that I wish to engage in eyelash extension services at your business of my own will.

  • Booking & Cancellation Policies

    • An active card on file is required to book all appointments. Removing the active card on file will disqualify you from attending or booking appointments until a new, valid card is supplied.
    • I ask that you give me at least 24-48 hours notice when cancelling or rescheduling so that I can adjust my schedule to accommodate other clients.
    • 50% of the service fee will be charged to the card on file when cancelling or rescheduling within 24 hours of your appointment.
    • 100% of the service fee will be charged to the card on file if cancelling or no-showing your appointment same day, or in the event of habitual same day cancellations/no-shows.

     

    After 3 cancellations or no-shows you will only be able to make appointments on a day-of basis and will be required to pay for the appointment at the time of booking.

    Habitually violating the booking and cancellation policies may result in being banned from booking appointments in advance and in some cases may result in being banned from booking entirely.

     

    All sales including services, gratuity, products and any  fees are final and non-refundable.
