Upload your Public Liability Insurance Document. The document should include all pages of the policy schedule, not just the certificate. It must show the words PUBLIC LIABILITY and the indemnity of £2m or more.

If you subscribe to the DBS Online Account service, you can grant temporary access for us to view your certificate online.

In order for us to access your information, simply complete the fields below. We'll need:

- The last 6 digits of your DBS certificate
- The 8 digit sharecode issued by DBS

You must upload a copy of the Asbestos Awareness Certificate (Dated within the last 12 months).

You are no longer required to submit a physical copy of your / your sub-contractor's electrical certification.

However, you must provide details of ALL electricans used, in order for us to verify their registration credentials.

Note that your electrician may be contacted to verify your relationship.

It is your LEGAL responsibility to comply with electrical regulation as definded by Part-P of The Building Regulations (England & Wales) and BS7671 and BS7691 (Scotland). It is your responsibility to check the credentials of all contractors used for electrical work, before commissioning work. Failure to do so may lead to prosecution.

It is your responsibility to BIFIS to ensure that details of all electricans used are declared using this form, either before work is commissioned or as reasonably possible thereafter.

However, you must provide details of ALL engineers used, in order for us to verify their registration credentials.

• CSCS registration number and surname - Provide us with your CSCS registration number and surname.
• CSCS Card - Picture of the front of your CSCS card
• Screenshot of CSCS App - Screenshot of your CSCS card from your CSCS app.

JIB cards with Domestic Home Appliance qualifications can be accepted for contract wet-fit roles only, and when requested by the retail partner.

Proof of HMRC registration is required for your registration, we can accept:

o CIS Card
o Letter of certificate from HMRC on official headed paper
o Letter from your accountant with your name and UTR (Unique Tax Reference) on official headed paper
o Payslip from an employer, dated within the last 5 years
