Dallas Elite Athletics Waiver/Release Policy

I hereby authorize the directors of Dallas Elite Athletics Camps to act for me according to their best judgment in any emergency requiring medical attention and I hereby release the camp, coaches, and facilities from any and all liability for any injuries or illness incurred while at DEA. I fully understand the cancellation policy described on DEA’s website. I fully grant DEA permission to use for publicity & advertising purposes all photographs or video taken of the athletes at any session.
