This agreement releases Christa Leigh’s School of Dance and Miliie's on Main from all liabilities relating to injuries that may occur during Dance Class. By signing this agreement, I agree to hold Christa Leigh’s School of Dance, and Millie's on Main entirely free from any liability. I also acknowledge the risks involved in Dancing. I swear that my child is participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my child’s likelihood of experiencing injuries while engaging in this activity.
By signing below I forfeit all right to bring a suit against Christa Leigh’s School of Dance for any reason. In return, my child will also make every effort to obey safety precautions and I will ask for clarification when needed.
