SEED AUTISM SERVICES makes every effort to care for your child but while it is our duty to maintain a safe environment for your child, we realise that accidents can occur. We cannot accept liability for any accidents which may occur at our premises.
Please thoroughly read, understand and agree to all our Terms and Conditions as attached prior to submission.
1. Admission Policies
1.1 There shall be a maximum of 5 children in each group. There are two groups : 5-8 years old, and 7-10 years old. SEED shall determine the suitability of the group for each child upon screening.
1.2 Admittance of children into the program is determined by SEED after the appropriate screening is complete and fulfills criteria set by the Lead Teacher.
1.3 Upon receiving the form, Parents will be contacted for further information and will be advised to either attend the screening or trial session. Please proceed with payment within 48 hours to confirm admission. In the event payment is not received within 48 hours, we have the rights to release the seat to another child in the waiting list.
2. Fees and Schedule
2.1 The table of fees is as per below.

2.2 Fees are to be fully paid at least seven (7) days before program commencement date.
2.3 Monthly fee is applicable for 4 sessions per month.
2.4 There shall be no sessions during National and Selangor Public Holidays as well as SEED's term break weeks as shall be advised.
2.5 If the child enrols in the program, the Screening Fee that was paid shall be used towards payment of registration fee.
2.6 Monthly fee payment must be paid in full within 10 days of invoice date. Non-payment within the due date will result in service suspension.
2.7 To recommence service that has been suspended, full payment of outstanding invoice and a re-commencement fee of RM 50 must be paid.
2.8 Service suspension shall be in effect for maximum 7 days. Service shall be immediately terminated and deposit forfeited if the required payment is not made during the suspension period.
2.9 Freezing of services is allowed for clients who have been on the program for a minimum of three (3) months, whereby the freeze request is made for not more than two (2) consecutive sessions per calendar year, at the rate of 50% of the Monthly fee.
2.10 Request to Freeze shall be made to the Program Lead prior to the freeze period. Classes that were missed shall not be eligible for freezing in retrospect.
3. Withdrawal / Deferment
3.1 A 20% charge will be made from the monthly fee if withdrawal is made prior to commencement of the program. No refund will be made for withdrawal after commencement of the program.
3.2 The deposit is refundable provided one (1) month’s written notice is served to SEED, via email to info@seedautism.my or billing@seedautism.my.
3.3 Refund, if any, will be deposited within 2 weeks upon receiving the notice and relevant bank information.
3.4 Deferment of program start date is allowed to the following month if the child is suddenly unable to attend the sessions in the current month.
3.5 Deferment is allowed under the condition that the fees have been fully paid.
3.6 Deferment is allowed one (1) time only per child.
4. Attendance & Pick Up
4.1 Regular attendance and punctuality is appreciated to ensure smooth running of the program.
4.2 In the event only one child is available to attend the class, it will proceed as scheduled, with individualized instruction and activities. No refunds or replacements shall be made for non-attendance.
4.3 Parents / Guardians will be responsible for their children until class start time. Please accompany your children if early.
4.4 Parents / Guardians are encouraged to be punctual when picking up their child from class. We understand that emergency situations may arise and we are willing to assist in such cases, however, these shall be treated as exceptions.
4.5 Parents / Guardians MUST notify the therapists if anyone other than the child’s primary caregiver will be picking up the child and are requested to provide the photo, name and IC number to the Lead Therapist for safety purposes.
5. Confidentiality
5.1 Any information concerning your child, which is provided or discussed with the Principal and staff members of SEED will remain confidential.
6. Attire and School Bag
6.1 Please dress your child up in comfortable attire which is loose fitting and lightweight.
6.2 Jewellery is discouraged.
6.3 Please ensure your child’s school bag contains:
a. Water bottle with water filled up
b. Snacks
c. Stationeries
d. A set of clean clothes
6.4 All items belonging to your child must be labelled with your child’s name.
7. Illnesses / Injuries
7.1 Please report any complications or illness to the therapist as soon as possible especially communicable diseases.
7.2 Sick children must be kept at home and therapist to be informed accordingly.
7.3 If any symptoms are apparent in a child, the parents / guardians will be called to pick up the child.
7.4 In case of emergency, the child will be taken directly to the nearest healthcare facility and parents / guardians will be contacted immediately in such an event.
7.5 In the interest of the children’s collective health, any child with a temperature of (37.5*C and above) will not be allowed to attend classes.
7.6 If your child has Hand Foot Mouth Disease (HFMD), it is notifiable to the centre by law.
8. Cancellations by SEED
8.1 SEED undertakes that there will always be a qualified therapist available to conduct the session.
8.2 In the unforeseeable circumstance that SEED cancels a session, a replacement class will be arranged.
9. Insurance
9.1 Every child attending the program shall be covered under the Tokio Marine Insurance Group Personal Accident Policy, for which the premium is valid for 12 months and its validity not affected by termination of the program.
10. Others
10.1 Food is not provided.
10.2 We reserve the right to review and amend the Rates, Terms and Conditions from time to time as required.

: hereby acknowledge that I have read, understood, agree and adhere to Seed Autism Services’ Terms and Conditions, and any standard policies and procedures that may be made known to me from time to time; and
: agree to release "SEED AUTISM SERVICES", including instructors and assistants from liability for any and all injuries which may occur during the program.
: give permission for the public sharing of any visual images that your child may appear in. However, SEED undertakes to blur out or hide the faces of children in those images hence making none of the children identifiable.
