    This form constitutes a commitment to the International Parkinson and Movement Disorder Society to support the 2023 AOPMC taking place March 17-19 in Kolkata. Upon submission of this form you will receive a confirmation letter from MDS along with a Letter of Agreement for review and signature, followed by an invoice and payment instructions.

  • Sponsor Levels

    MDS will recognize sponsors of the 8th AOPMC based on their cumulative sponsorship, exhibit and advertising spend. The sponsorship levels are:

    • $60,000+ USD                   Platinum
      (company logo included in recognition + 15 full AOPMC registrations)
    • $40,000 - $59,999 USD     Gold
      (company logo included in recognition + 10 full AOPMC registrations)
    • $25,000 - $39,999 USD     Silver
      (company logo included in recognition + 5 full AOPMC registrations)
    • $10,000 - $24,999 USD     Bronze
      (company logo included in recognition)
    • Under $10,000 USD          Supporter
      (company name listed in recognition)

    Confirmation
    All requested opportunities are not considered guaranteed until MDS issues a confirmation to your company. Items are guaranteed on a first-come, first-served basis. Should there be more than one sponsor interested in the same sponsorship opportunity, priority will be given by date of receipt of this form.

  • Payment Information
    Specific directions for payment of sponsorship and/or exhibit support will be provided upon commitment and accompanied by an invoice. All amounts due are to be paid in US Dollars.

    Cancellation Policy
    For Bronze Level Sponsors and above, cancellation requests must be sent to MDS in writing at development@movementdisorders.org. A full refund, less a $1,000 USD administrative fee, will be given if notice is received by January 6, 2023. No refund will be given after January 6, 2023. All other cancellation/refund requests will be made on a case-by-case basis.

  • By completing this form, the company representative attests they have read and understand the Sponsor and Exhibitor Regulations included in the Sponsorship Prospectus and are confirming the sponsorship items indicated in this form, pending reciept of a confirmation letter from the MDS Secretariat.
