Acknowledgment and Authorization for Criminal Background Check

As a condition of my candidacy as independent contractor, volunteer or employee with Global Service Network, I understand that the organization will conduct a criminal background check and/or a credit report about me for employment purposes. By signing this Acknowledgment and Authorization, I authorize Global Service Network and Volunteer Select Plus, or any other company authorized by the organization, to access such information as may be necessary to complete a criminal background check and/or credit report. I release from liability all persons and entities supplying such information. I indemnify Global Service Network and Volunteer Select Plus, or any other company authorized by Global Service Network, against any liability that may result from making such requests. I agree that a fax, scan or photocopy of the Acknowledgment and Authorization with my signature will be accepted with the same authority as the original. I understand that upon my request, I will be given a copy of the background report and a written description of my rights under the Fair Credit Reporting Act.
