  • Please fill out all of the sections to the best of your ability. I will not accept incomplete forms. Fill out one form per person. If you wish to book a matching tattoo or book the same time as a friend, please each fill up a form and then put your friend's name in the notes section at the end.

    Please note I DO NOT tattoo anyone under the age of 18. 

    I DO NOT tattoo feet, face, throat, or any genital areas.

    I will review each request and reach out via. email within 3 weeks.

    Please note that once your appointment request is accepted, I will reach out to you via email with your appointment date/time. You will be required to pay a deposit at that time to secure the deposit. Instructions on how to pay the deposit will be given with your appointment information. If the deposit is not recieved witin 48 hours, your appointment slot will go to someone else. 

    Deposits are nonrefundable but can be transferred to another time slot if you reschedule with at least 48 hours notice. If you do not provide 48 hours notice of rescheduling, your deposit will be automatically forfeit.
