Volunteer One-Event Form

Thank you for choosing to volunteer with Boys & Girls Clubs of Metro Los Angeles (BGCMLA)! Please complete and submit the form below prior to volunteering onsite at our Clubhouses. 

*Due to COVID-19, we are accepting a limited number of volunteers.

Please read and sign below. 

• I understand my involvement in activities with Boys & Girls Clubs of Metro Los Angeles (BGCMLA) and its partner agencies is voluntary, and I will receive no compensation or remuneration from the Club for the work that I will perform.
• I understand that this release is for a one-time event and I will contact my BGCMLA representative or the Volunteer Coordinator if I would like to volunteer again.
• I understand that volunteering may involve the use of tools, working inside or outside buildings, working in a variety of weather conditions, standing and walking on uneven terrain, and performing work that may be repetitive, or require lifting, sitting, stooping, and/or stretching.
• In connection with my voluntary involvement in activities undertaken for and with the participation and support of BGCMLA, a nonprofit charitable organization, I hereby agree for myself, my heirs, assignees, executors, and administrators to release and discharge BGCMLA, its officers, and directors, employees, agents, all partner agencies, and volunteers from all claims, demands, and actions for injuries sustained to my person and/or property as a result of my involvement in such activities.
• I agree to release and hold BGCMLA, its officers and directors, employees, agents, all partner agencies, and volunteers harmless from any cause of action, claim, or suit arising therefrom.
• I hereby attest that my attendance and involvement in such activities is voluntary, that I am participating at my own risk, and that I have read the foregoing terms and conditions or this release.
• I agree to the use of my photograph for the publicity and public relations of BGCMLA, and its partner agencies.
• I hereby confirm, represent and warrant that I have never been convicted or charged with a violent crime, child abuse, or neglect, child pornography, child abduction, kidnapping, rape, or any sexual offense, nor have I been ordered by a court to receive psychiatric or psychological treatment in connection therewith.
• I agree to never hug or touch youth, allow them to sit on my lap, hold hands, etc.
• I agree that I will not take pictures of BGCMLA members without written consent from BGCMLA.
• I agree to not enclose myself in a room alone with any BGCMLA member or allow BGCMLA members in my vehicle.
• I agree to abstain from outside interaction with BGCMLA members outside of BGCMLA property and/or BGCMLA hours.

Relating to Coronavirus/COVID-19

• I agree to assume all of the foregoing risks and accept sole responsibility for any injury (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my continued work or volunteerism onsite at the Club and/or non-remotely (“Claims”). I hereby release, covenant not to sue, discharge, and hold harmless the Boys & Girls Clubs of Metro Los Angeles, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating to it. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Club, its employees, agents, and representatives, whether an injury, loss or COVID-19 infection occurs before, during, or after working onsite at the Club and/or non-remotely during this time.
