Student-athletes participating in Volleyball, Basketball, or Track will be required to participate in fund-raising during the school year. Each student-athlete will be required to meet invidicual and team fund-raising goals. Details of these events will be shared with the student-athletes and their families.

Parent Consent & Info: By completing and signing this form, I give my consent to have my child sign-up for and participate in the sports selected. I also agree to all that is provided and required for participation.
1. My child will have a Sports Physical/Waiver completed and turned into the school before participating in any school sports.

2. I understand that sports are physically exerting and I give consent to have my child participate in such an activity.

3. I realize athletic activities can pose some risk for injury and I give consent to have my child participate. The school will act to the best of their ability to keep my child safe, but I will not hold the school responsible for injuries that result from activities/practices/games that take place as a result of the sport I have allowed my child to participate in.

4. I understand that my child will not be allowed to participate in games/performances if they are not at mandatory practices and/or they are not currently meeting academic requirements set forth in the Athletic Conduct Packet. I also understand my student may be immediately expelled from the team if they fail to meet the code of conduct set forth (portraying themselves in a Biblical manner—showing respect and honoring God).

I acknowledge receipt of the FCS Athletic Packet, have fully read through and agree to follow all of the information, procedures and rules set forth as well as give permission for my child to fully participate in the sports designated.
