Read the following information and check the box to indicate your agreement. I, the undersigned, do hereby agree to participate in or allow the individual named herein to participate in the aforementioned activity. I assume all risks and liability that may arise from my or my child's involvement and participation in this activity. I understand that this program carries the possibility of physical injury and may involve physical activity that may be strenuous and there are risks inherent in this recreational activity. With regard to the activity to which this form applies, nothing shall be construed to grant an expressed or implied warranty of safety. I further understand that Hidden Valley Youth Sports Association and its officers, agents, and volunteers are not liable for any injury that may result from the negligence of persons conducting this program. Hidden Valley Youth Sports Association recommends that participants secure adequate medical insurance to cover any injury that may arise from participation in Hidden Valley Youth Sports Association's recreation programs. We (Player & Parents/Guardians) further agree to abide by ALL Hidden Valley Youth Sports Association rules and to conduct ourselves in a sportsmanlike manner during all Hidden Valley Youth Sports Association related activities. We further agree to abide by all directions and decisions of the referees, coaches, and officials of the Hidden Valley Youth Sports Association Program. We realize that violations of the above are consequential and could result in our suspension for one (1) or more games. Equipment Return: All equipment is property of the Hidden Valley Youth Sports Association, and must be returned within 2 weeks of the season's completion. It is the parents responsibility to return equipment to the coach. You acknowledge and authorize the HVYSA to charge you for equipment which is lost, missing, damaged through misuse, or not turned in on time.
Minor Child Photo Release Waiver

I grant HVYSA permission to use photographs taken of my child during practices and games, for any legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content. Furthermore, I understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
