  • SECTION 9: Final Application Requirements and Attachments - Checklist

    It is highly recommended and preferred that the applicant submit an electronic copy of the full application to MaldenCPC@cityofmalden.org through this webform. All attachments shall be uploaded here. Alternatively, you may submit 2 paper copies of the application and any attachments which must be time-stamped upon receipt by the City Clerk (Malden City Hall, 215 Pleasant Street, 2nd Floor - Room 220, Malden, MA 02148). The application should be submitted unbound.

  • SUBMITTAL INSTRUCTIONS

    If you are not finished with your application or need to share with another party for signature or more information or attachments: select "Save" before selecting "Submit", and you will receive an email with a link to edit your form. You can then share this link others who are reviewing your draft, including the property owner who must sign the application in Section 8.

    Once all fields are complete and all relevant attachments have been uploaded: Open the link and click “Submit”. You should receive a confirmation email.
