Tattoo Request Form

My name is Sadie (Tatty_Feather) and I’m a handpoke tattoo artist based in Somerset & Hampshire. I have just opened my own little private studio in the heart of Wiveliscombe and also occasionally work in a beautiful private treatment space located in Liphook. This form will make the process of booking your tattoo smooth and easy for the both of us. Please fill in my questions for you and if you have any yourself there is a space for them on the last page. Every person & body is sooo welcome in my studio all I ask is that you are the legal age of 18+ years. I work in black, blue and brown tattoo ink which is vegan and the highest pigment quality available. You will be asked for a non-refundable deposit of £50 to secure your tattoo appointment. This goes towards the final price of the tattoo. You can move your appointment only once without losing this deposit and only if you contact me at least 48 hours before your slot. (I would appreciate if you could let me know as soon as possible so that I can offer your slot to someone else, thank you!) To give you an idea about pricing- Minimum charge is £80. If you’re looking to have tiny tattoos it is best to book them in all together. Most of the tattoos you see on my Instagram are priced £80-300. I can give you a quote which will be dependent on the idea, body placement and size. If I don’t think your idea is suited to handpoke or my style I will be very honest with you and try to recommend a different artist. If you’d like to book multiple tattoos in one session, please include information for all of them. If you’d like to book in with a friend(s) you can fill one form for both/all of you with all details and explain it at the bottom in an additional question box. I try to use all vegan supplies and as many eco-conscious products throughout the tattoo process as available.
